The Outreach Coordinator is responsible for developing and maintaining strategic relationships that increase patient referrals and drive the clinic's Average Daily Census (ADC). This role focuses on expanding referral networks, strengthening partnerships with community stakeholders, and ensuring an effective referral-to-intake conversion process.

The Outreach Coordinator will actively engage community organizations, healthcare providers, government agencies, and recovery-focused networks to promote access to treatment services and increase patient engagement. This role requires strong relationship-building skills, data tracking, and the ability to communicate the intake and referral process effectively.

In addition to supporting a variety of outreach initiatives to build systems of care for patients to access treatment throughout Hope and Healing Clinic's service area, they will work as part of a multidisciplinary team to increase awareness around substance use disorder, with an emphasis on opioid use, create an education campaign, and connect potential patients to Hope and Healing Clinic's Opioid Treatment Program. This position will report directly to the Clinic Administrator.

Essential Job Duties

Census Growth &amp; Referral Development
<ul>
<li>Drive average daily census growth through targeted outreach and relationship development.</li>
<li>Identify, develop, and maintain strong relationships with key referral sources defined by leadership.</li>
<li>Grow and diversify the clinic's referral network including hospitals, behavioral health providers, community organizations, and public agencies.</li>
<li>Develop and implement a structured referral process to streamline partner engagement and improve patient intake efficiency.</li>
<li>Track and monitor conversion rates from referral to admission based on outreach activities.</li>
<li>Meet or exceed outreach quotas and performance benchmarks tied to census growth.</li>
</ul>
Community Relationship Development
<ul>
<li>Build and maintain professional relationships with local service organizations, hospitals, law enforcement, courts, public health agencies, and social service providers.</li>
<li>Serve as the clinic's community liaison, attending events and meetings to educate partners on services and treatment pathways.</li>
<li>Support coalitions, task forces, and community initiatives aimed at strengthening addiction recovery resources and support networks.</li>
<li>Engage with local government bodies, city and county administrators, and community leadership to promote treatment access and collaboration.</li>
</ul>
Outreach Strategy &amp; Engagement
<ul>
<li>Develop outreach strategies targeting high-need populations including:</li>
<li><ul><li>Homeless individuals</li>
<li>Pregnant and parenting women</li>
<li>Individuals who use IV drugs</li>
<li>Individuals involved in the criminal justice system</li>
</ul></li><li>Conduct outreach at locations such as:</li>
<li><ul><li>Homeless camps and shelters</li>
<li>Emergency departments</li>
<li>Detox centers</li>
<li>Recovery programs</li>
<li>Community resource centers</li>
</ul></li><li>Participate in community events and public education initiatives to increase awareness of treatment services.</li>
</ul>
Referral Process &amp; Intake Communication
<ul>
<li>Clearly communicate the clinic's intake and referral process to partners and potential patients.</li>
<li>Develop an understanding of partner referral processes to improve coordination and patient access to care.</li>
<li>Collaborate with internal teams to ensure smooth transition from referral to patient admission.</li>
</ul>
Marketing &amp; Thought Leadership
<ul>
<li>Assist with marketing initiatives and partnership development to increase program awareness.</li>
<li>Contribute to the development of presentations, educational materials, and outreach content to support community engagement.</li>
<li>Identify opportunities to increase patient retention through engagement strategies and community support networks.</li>
</ul>
Data Tracking &amp; Performance Monitoring
<ul>
<li>Track outreach activity metrics and report performance outcomes including:</li>

</ul>
Operational &amp; Compliance Responsibilities
<ul>
<li>Maintain regular communication with the Management Services Organization (MSO) and attend meetings as requested.</li>
<li>Ensure full compliance with HIPAA policies and patient confidentiality standards.</li>
<li>Ensure adherence to all clinic policies, procedures, and operational standards.</li>
<li>Recommend improvements to policies and procedures when appropriate.</li>
<li>Protect the rights and confidentiality of all patients while supporting delivery of high-quality clinical care.</li>
</ul>
Team Collaboration
<ul>
<li>Interact with all staff in a positive, professional, and motivational manner that supports the organization's mission and patient-centered care model.</li>
<li>Collaborate with clinical, operational, and administrative teams to support patient access and continuity of care.</li>
<li>Participation in clinic team morale building events.</li>
</ul>
Minimum Requirements
<ul>
<li>2-3 Years successful experience planning and executing strategic initiatives in a healthcare/clinic environment</li>
<li>Valid driver's license, proof of insurance and clean Motor Vehicle Record</li>
</ul>
Preferred Requirements:
<ul>
<li>Bachelor's degree in healthcare administration, public health, marketing, social services, or related field</li>
</ul>

<li>Opioid Treatment Program experience will be given hiring preference</li>

Hope and Healing Clinic is seeking a Mobile Unit Medical Driver who is eager to support their clinic located in Tumwater, WA.

The Mobile Unit Medical Driver will provide driver services to the Clinic to include setup, breakdown, minor maintenance, operational troubleshooting, coordination of maintenance/repair activities and provide support services for the mobile clinic. This role is a community based role, and is responsible for maintaining a safe, secure, and welcoming environment while supporting both security operations and front-line clinic services. The position integrates safety, transportation, mobile unit facility oversight, and member-facing responsibilities to ensure efficient and compliant clinic operations.

Responsibilities

Safety &amp; Security:
<ul>
<li>Maintain a secure environment to protect patients, staff, visitors, property, and sensitive information.</li>
<li>Proactively identify, prevent, and respond to safety risks, suspicious behavior, and emergency situations, including workplace violence and crisis events.</li>
<li>Conduct routine patrols, inspections, and surveillance of buildings, grounds, parking lots, and mobile units to ensure compliance with safety and fire regulations.</li>
<li>Utilize de-escalation and conflict resolution techniques; respond to incidents with professionalism, compassion, and adherence to policy.</li>
<li>Complete timely and accurate incident reports and collaborate with leadership, law enforcement, and first responders as needed.</li>
<li>Ensure compliance with all regulatory requirements (HIPAA, DEA, FDA, state/local agencies) and protect patient confidentiality.</li>
</ul>
Transportation &amp; Mobile Unit Operations:
<ul>
<li>Safely operate company vehicles for patient transport and clinic-related activities.</li>
<li>Maintain operational readiness, cleanliness, and safety of vehicles and mobile unit(s), including basic maintenance, troubleshooting, and coordination of repairs.</li>
<li>Ensure compliance with all vehicle regulations and complete required documentation.</li>
<li>Assist with medication delivery routes and transportation logistics when needed.</li>
</ul>
Facility &amp; Environmental Oversight:
<ul>
<li>Maintain clean, organized, and safe environments across clinic spaces, waiting areas, and parking lots.</li>
<li>Monitor and restock sanitation stations and promote infection control practices.</li>
<li>Support crowd control and ensure adherence to clinic policies, including safety and social distancing guidelines.</li>
</ul>
Member &amp; Front-Line Support:
<ul>
<li>Greet and assist patients, visitors, and vendors with professionalism and a service-oriented approach.</li>
<li>Support front desk operations, including managing phone lines, facilitating member flow, assisting with check-in, verifying insurance, and processing payments.</li>
<li>Handle medical records requests and mail distribution in compliance with privacy standards.</li>
<li>Provide directions, information, and hands-on assistance to enhance the member experience.</li>
</ul>
Collaboration &amp; Communication:
<ul>
<li>Build strong working relationships with internal teams and external partners through liaison rounding and outreach.</li>
<li>Provide security awareness education and support staff in maintaining a safe environment.</li>
<li>Participate in staff meetings, trainings, and ongoing professional development, including CPR, de-escalation, and safety protocols.</li>
<li>May assist in training and mentoring new staff.</li>
</ul>
Operational Excellence:
<ul>
<li>Support department goals by adhering to policies, procedures, and quality standards.</li>
<li>Monitor workflows, contribute to process improvements, and ensure efficient daily operations.</li>
<li>Perform additional duties as assigned to support clinic and mobile unit success.</li>

Overall, this role combines security, transportation, facility management, and member services to deliver a safe, compliant, and high-quality patient experience.

 Position Requirements

 Education/Experience:
<ul>
<li>1yr or more Opioid Treatment Program or Healthcare experience.</li>
<li>Must have a clean (DMV) driving record with no violations in the past 3 years.</li>
<li>Must be willing to work outdoors in all weather conditions.</li>
<li>Experience with regular and ongoing insurance verification, including experience troubleshooting denials.</li>
<li>Experience using de-escalation and safety techniques in a healthcare environment</li>
</ul>
Preferred

Fleet requirements:
<ul>
<li>Experience with minor mechanical repair and maintenance troubleshooting on RV or large mobile vehicles.</li>
</ul>
 Physical and Environmental Requirements:
<ul>
<li>Must be able to lift from the floor to overhead levels and carry items at waist level weight up to 70 pounds.</li>
<li>Must be able to stand or sit for extended periods.</li>
<li>Requires the ability to stoop, bend, reach, crouch, and squat.</li>
<li>Requires full body range of motion including ability to reach from floor to overhead levels, manual and finger dexterity, and eye-hand coordination.</li>
<li>Must be willing to work in multiple clinic settings, indoors and out, which may result in exposure to some communicable disease and a variety of weather conditions.</li>
<li>Must be able to operate patient lifting equipment and patient handling devices accurately and safely under the direction of the patient care staff.</li>
</ul>

Must work closely with the VP of Operations, Clinic Administrator, Medical Director, Dispensary Manager, Continuous Improvement Manager, and various stakeholders to identify areas in need of improvement, training opportunities, strategize mobile unit operational changes, and make recommendations utilizing a variety of data points while maintaining patient care as a priority.</li>
<li>Must be computer literate, with intermediate knowledge of Microsoft Office Suite, and electronic medical records systems, as well as a high level of initiative in keeping current with technological change.</li>
