The Outreach Coordinator is responsible for developing and maintaining strategic relationships that increase patient referrals and drive the clinic's Average Daily Census (ADC). This role focuses on expanding referral networks, strengthening partnerships with community stakeholders, and ensuring an effective referral-to-intake conversion process.
The Outreach Coordinator will actively engage community organizations, healthcare providers, government agencies, and recovery-focused networks to promote access to treatment services and increase patient engagement. This role requires strong relationship-building skills, data tracking, and the ability to communicate the intake and referral process effectively.
In addition to supporting a variety of outreach initiatives to build systems of care for patients to access treatment throughout Hope and Healing Clinic's service area, they will work as part of a multidisciplinary team to increase awareness around substance use disorder, with an emphasis on opioid use, create an education campaign, and connect potential patients to Hope and Healing Clinic's Opioid Treatment Program. This position will report directly to the Clinic Administrator.
Essential Job Duties
Census Growth & Referral Development
- Drive average daily census growth through targeted outreach and relationship development.
- Identify, develop, and maintain strong relationships with key referral sources defined by leadership.
- Grow and diversify the clinic's referral network including hospitals, behavioral health providers, community organizations, and public agencies.
- Develop and implement a structured referral process to streamline partner engagement and improve patient intake efficiency.
- Track and monitor conversion rates from referral to admission based on outreach activities.
- Meet or exceed outreach quotas and performance benchmarks tied to census growth.
Community Relationship Development
- Build and maintain professional relationships with local service organizations, hospitals, law enforcement, courts, public health agencies, and social service providers.
- Serve as the clinic's community liaison, attending events and meetings to educate partners on services and treatment pathways.
- Support coalitions, task forces, and community initiatives aimed at strengthening addiction recovery resources and support networks.
- Engage with local government bodies, city and county administrators, and community leadership to promote treatment access and collaboration.
Outreach Strategy & Engagement
- Develop outreach strategies targeting high-need populations including:
- Homeless individuals
- Pregnant and parenting women
- Individuals who use IV drugs
- Individuals involved in the criminal justice system
- Conduct outreach at locations such as:
- Homeless camps and shelters
- Emergency departments
- Detox centers
- Recovery programs
- Community resource centers
- Participate in community events and public education initiatives to increase awareness of treatment services.
Referral Process & Intake Communication
- Clearly communicate the clinic's intake and referral process to partners and potential patients.
- Develop an understanding of partner referral processes to improve coordination and patient access to care.
- Collaborate with internal teams to ensure smooth transition from referral to patient admission.
Marketing & Thought Leadership
- Assist with marketing initiatives and partnership development to increase program awareness.
- Contribute to the development of presentations, educational materials, and outreach content to support community engagement.
- Identify opportunities to increase patient retention through engagement strategies and community support networks.
Data Tracking & Performance Monitoring
- Track outreach activity metrics and report performance outcomes including:
- Referral volume
- Conversion rates
- Outreach engagement metrics
- Community partner growth
- Use outreach data to refine strategies and improve program effectiveness.
Operational & Compliance Responsibilities
- Maintain regular communication with the Management Services Organization (MSO) and attend meetings as requested.
- Ensure full compliance with HIPAA policies and patient confidentiality standards.
- Ensure adherence to all clinic policies, procedures, and operational standards.
- Recommend improvements to policies and procedures when appropriate.
- Protect the rights and confidentiality of all patients while supporting delivery of high-quality clinical care.
Team Collaboration
- Interact with all staff in a positive, professional, and motivational manner that supports the organization's mission and patient-centered care model.
- Collaborate with clinical, operational, and administrative teams to support patient access and continuity of care.
- Participation in clinic team morale building events.
Minimum Requirements
- 2-3 Years successful experience planning and executing strategic initiatives in a healthcare/clinic environment
- Valid driver's license, proof of insurance and clean Motor Vehicle Record
Preferred Requirements:
- Bachelor's degree in healthcare administration, public health, marketing, social services, or related field
Additional:
- The nature of this position is fast-paced, at times high-stress situations and conditions
- Opioid Treatment Program experience will be given hiring preference
- Have planned and executed strategic initiatives successfully
- Extensive knowledge of community resources
- Extensive knowledge of behavioral health challenges which include knowledge of the impacts of mental illness, domestic violence, and the dynamics of addictive behavior
- Computer literate, with basic knowledge of Microsoft Office Suite, and electronic medical records systems, as well as a high level of initiative in keeping current with technological change
- Ability to establish and maintain effective working relationships with staff, clients, and outside contacts from a wide variety of ethnic, socioeconomic, and cultural backgrounds; good diplomatic skills, including strong conflict-resolution skills
- Excellent interpersonal and communication skills
- Excellent organizational and time management skills
- Conducts all business activities in a professional and ethical manner
- No history of alcohol or other drug misuse for 3 years prior to employment and does not display evidence of alcohol or other drugs while employed as the OTP Clinic Director
- Must be able to pass required background checks
- Complies with and up to date with all other testing, screening and credentialing as required
- Travel required
