Job Overview We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. Your expertise will support a diverse patient in home health, and community care settings. This position offers an exciting opportunity to make a meaningful difference in patients&rsquo; lives through expert physiotherapy interventions and compassionate care.Duties<ul><li>Conduct comprehensive patient evaluations to determine functional limitations and develop tailored care plans that align with individual goals and medical conditions.</li><li>Implement evidence-based physiotherapy techniques, including trigger point therapy, manual therapy, and exercise prescriptions to promote recovery.</li><li>Manage discharge planning by coordinating with multidisciplinary teams to ensure seamless transition from hospital or clinic settings to home or community environments.</li><li>Provide specialized care for diverse populations including geriatrics, orthopedics, and patients recovering from acute care or hospital stays.</li><li>Document patient progress accurately using electronic medical record (EMR) systems while maintaining detailed records of treatments administered and patient responses.</li><li>Educate patients and caregivers on physiotherapy exercises, injury prevention strategies, and home care routines to support ongoing recovery.</li><li>Stay current with advancements in physiology knowledge, medical terminology, and physiotherapy techniques to deliver the highest standard of care.</li></ul>Skills<ul><li>Strong knowledge of anatomy and physiology principles underpinning effective physiotherapy interventions.</li><li>Proven experience in inpatient hospital environments, outpatient clinics, home health services, or community care programs.</li><li>Proficiency with EMR systems for documentation and treatment planning.</li><li>Excellent patient care skills with the ability to communicate complex concepts clearly and empathetically.</li><li>Familiarity with discharge planning processes and collaborative care approaches across healthcare disciplines.</li><li>Experience in geriatrics, and orthopedics, is highly desirable.</li><li>Ability to perform trigger point therapy and other manual techniques effectively.
