Led by experienced physicians specializing in inpatient management, the organization offers robust support to ensure excellence in hospital-based care.Job Summary We are seeking a highly skilled and motivated Advanced Practice Provider (NP/PA) specializing in post-acute care to join our team in Hemet /Menifee, CA. The successful candidate will play a vital role in coordinating patient recovery and transition from hospital to community settings. This position offers an opportunity to deliver comprehensive, patient-centered care in a post-acute environment, utilizing your clinical expertise and passion for improving patient outcomes.Duties<ul><li>Provide comprehensive patient care in post-acute settings</li><li>Conduct routine rounding, assessments, and follow-ups</li><li>Collaborate with physicians, nursing staff, and interdisciplinary teams</li><li>Manage chronic conditions and oversee care transitions</li><li>Ensure accurate and timely documentation in EMR</li><li>Communicate effectively with patients and families.</li></ul>Experience<ul><li>Licensed NP or PA (California license or ability to obtain)</li><li>Prior post-acute care experience required (SNF, LTACH, rehab, or similar)</li><li>Strong clinical judgment and ability to work independently</li><li>Comfortable managing a consistent patient panel</li><li>Excellent communication and team collaboration skills</li></ul>Why This Role<ul><li>Predictable weekday schedule &mdash; no weekends required</li><li>Low-burden call structure (not disruptive to lifestyle)</li><li>Stable, long-term position with a growing organization</li><li>Ideal for providers who prefer continuity of care over high-turnover settings</li></ul>Pay: From $140,000.00 per yearBenefits:<ul><li>401(k)</li><li>401(k) matching</li><li>Dental insurance</li><li>Health insurance</li><li>Health savings account</li><li>Life insurance</li><li>Paid time off</li><li>Relocation assistance</li><li>Vision insurance</li></ul>Work Location: In person
