Job Summary
We are seeking a highly skilled and motivated Advanced Practice Provider (NP/PA) specializing in post-acute care to join our team in Hemet /Menifee, CA. The successful candidate will play a vital role in coordinating patient recovery and transition from hospital to community settings. This position offers an opportunity to deliver comprehensive, patient-centered care in a post-acute environment, utilizing your clinical expertise and passion for improving patient outcomes.

Duties

• Provide comprehensive patient care in post-acute settings
• Conduct routine rounding, assessments, and follow-ups
• Collaborate with physicians, nursing staff, and interdisciplinary teams
• Manage chronic conditions and oversee care transitions
• Ensure accurate and timely documentation in EMR
• Communicate effectively with patients and families.

Experience

• Licensed NP or PA (California license or ability to obtain)
• Prior post-acute care experience required (SNF, LTACH, rehab, or similar)
• Strong clinical judgment and ability to work independently
• Comfortable managing a consistent patient panel
• Excellent communication and team collaboration skills
