Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA) to join our healthcare team. Monday - Thursday 8:00am- 4:30pm and Fridays 8:00am-12:00pm. We are closed on all major Holidays. No weekends. This role involves providing high-quality patient care across various settings, including acute care, long-term care, outpatient clinics, and community health environments. The ideal candidate will possess a strong foundation in medical procedures, excellent patient assessment skills, and experience working with diverse patient populations, including seniors and individuals with disabilities. This position offers an opportunity to work in a dynamic healthcare environment committed to delivering exceptional patient-centered care.Responsibilities<ul><li>Perform comprehensive patient assessments including vital signs, physical examinations, and medical history documentation</li><li>Administer medications and injections accurately following physician orders and safety protocols</li><li>Provide specialized care for patients with complex needs such as dementia, Alzheimer&rsquo;s disease, hospice care, or acute pain management</li><li>Support discharge planning and coordinate follow-up care for outpatient and inpatient populations</li><li>Document all patient interactions thoroughly within EMR/EHR systems like Epic or Aprima</li><li>Educate patients and families on health management, medication adherence, and post-care instructions</li><li>Ensure compliance with HIPAA regulations and infection control standards at all times</li><li>Refill medications</li></ul>Skills<ul><li>Proven experience in primary care or home &amp; community care</li><li>Proficiency with EMR/EHR systems including Epic, Aprima</li><li>Strong knowledge of medical terminology</li><li>Skilled in patient assessment techniques including vital signs measurement and physical examinations</li><li>Ability to perform venipuncture, phlebotomy</li><li>Experience administering injections and managing ventilators or respiratory support devices</li><li>Excellent communication skills for working with patients</li><li>Knowledge of infection control practices and aseptic techniques to prevent healthcare-associated infections</li><li>Ability to work efficiently under pressure in high-acuity environments</li></ul>This position requires candidates to pass a background check and drug screening prior to employment.
