Your energetic approach and compassionate mindset will empower clients to achieve greater independence and stability while ensuring compliance with Medicaid waiver programs.Responsibilities<ul><li>Conduct thorough assessments of clients&rsquo; needs.</li><li>Develop personalized care plans that align with Medicaid waiver guidelines and incorporate evidence-based practices such as motivational interviewing and crisis management strategies.</li><li>Collaborate with healthcare providers, social service agencies, and community organizations to coordinate services and facilitate access to resources like housing, employment, and mental health treatment.</li><li>Monitor client progress regularly, adjusting care plans as necessary to promote recovery and independence.</li><li>Educate clients on available programs such as Section 8 housing assistance and workforce development initiatives to enhance their self-sufficiency.</li><li>Document all interactions, assessments, and service plans accurately within case management systems while maintaining confidentiality.</li></ul>Qualifications<ul><li>Proven experience or training in behavioral health, social work, or related fields such as addiction counseling or crisis management.</li><li>Knowledge of Medicaid waiver programs, including Section 8 housing assistance and workforce development initiatives.</li><li>Strong skills in motivational interviewing techniques to engage clients effectively.</li><li>Ability to handle sensitive situations with empathy while managing crises professionally.</li><li>Excellent communication skills to collaborate with diverse teams and advocate for clients&rsquo; needs.</li><li>Familiarity with community resources related to behavioral health, housing, employment, and social services.</li><li>Bachelor's degree in social work or related field is required.</li></ul>Pay: From $25.00 per hourWork Location: In person
