Responsibilities
- Conduct thorough assessments of clients’ needs.
- Develop personalized care plans that align with Medicaid waiver guidelines and incorporate evidence-based practices such as motivational interviewing and crisis management strategies.
- Collaborate with healthcare providers, social service agencies, and community organizations to coordinate services and facilitate access to resources like housing, employment, and mental health treatment.
- Monitor client progress regularly, adjusting care plans as necessary to promote recovery and independence.
- Educate clients on available programs such as Section 8 housing assistance and workforce development initiatives to enhance their self-sufficiency.
- Document all interactions, assessments, and service plans accurately within case management systems while maintaining confidentiality.
Qualifications
- Proven experience or training in behavioral health, social work, or related fields such as addiction counseling or crisis management.
- Knowledge of Medicaid waiver programs, including Section 8 housing assistance and workforce development initiatives.
- Strong skills in motivational interviewing techniques to engage clients effectively.
- Ability to handle sensitive situations with empathy while managing crises professionally.
- Excellent communication skills to collaborate with diverse teams and advocate for clients’ needs.
- Familiarity with community resources related to behavioral health, housing, employment, and social services.
- Bachelor's degree in social work or related field is required.
Pay: From $25.00 per hour
