We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will have a strong background in special education, patient care, and behavioral health, with a focus on supporting individuals with developmental disabilities. Qualifications
- Bachelor's degree or higher in ABA/Psychology/ Social Work or related field
- Previous experience working with individuals with developmental disabilities
- Strong program management skills
- Excellent relationship management abilities
Duties
- Provide case management services to individuals with developmental disabilities
- Develop and implement individualized service plans
- Coordinate with external agencies to ensure comprehensive care for clients
- Conduct assessments to identify client needs and goals
- Monitor client progress and adjust service plans as needed
- Collaborate with a multidisciplinary team to address client needs
- Manage program budgets and resources effectively
