ABA Case Supervisor ($25-$35)

Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will have a strong background in special education, patient care, and behavioral health, with a focus on supporting individuals with developmental disabilities. As a Case Manager, you will play a crucial role in assessing patient needs, developing care plans, and coordinating services to ensure the best outcomes for our clients.

Responsibilities

• Conduct comprehensive patient assessments to identify needs and develop individualized care plans.
• Collaborate with healthcare providers, families, and community resources to coordinate services.
• Maintain accurate documentation of patient progress and treatment plans in compliance with HIPAA regulations.
• Collect and analyze data related to patient care and outcomes.
• Provide support and guidance to patients and their families regarding available resources and services.
• Advocate for patients' rights and needs within the healthcare system.
• Monitor patient progress and adjust care plans as necessary to ensure effective treatment.
• Stay informed about developments in medical terminology and best practices related to behavioral health.

Qualifications

- Bachelor's degree or higher in ABA/Psychology/ Social Work or related field
- Previous experience working with individuals with developmental disabilities
- Strong program management skills
- Excellent relationship management abilities

Duties
- Provide case management services to individuals with developmental disabilities
- Develop and implement individualized service plans
- Coordinate with external agencies to ensure comprehensive care for clients
- Conduct assessments to identify client needs and goals
- Monitor client progress and adjust service plans as needed
- Collaborate with a multidisciplinary team to address client needs
- Manage program budgets and resources effectively
