POSITION OVERVIEW: The Case Management Specialist supports residents at Les Marquis House move toward self-sufficiency and secure a more permanent place to call home. The Case Management Specialist strives to provide a high level of customer service to our residents at all times and executes operational tasks while on site. The goal of the Case Management Specialist is to ensure the security and safety of the site and its residents, carry out program guidelines and schedules, handle crisis situations, and assist residents as needed.
We are currently seeking candidates for the following shifts (Note: Schedule may vary based on program need): Monday and Tuesday 8AM - 4PM Wednesday and Thursday 4PM - 12AM

ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:
Case Management<ul><li>

Case Management Specialist

POSITION OVERVIEW:
The Case Manager will receive one hour of individual supervision per week, and take part in a two-hour group supervision. The Case Manager will regularly perform assessments of clients&rsquo; needs. Provide support and guidance to other staff within the Caminos program, including: clarifying program procedures, aiding in onboarding of new staff members, and</li><li>
Perform regular assessments of clients' needs in the following areas: housing, daily activities, counseling, coordination of other care, social relationships, family relationships, symptom management, securing and managing entitlements such as SSI, budgeting, and other related areas.</li><li>
Develop collaborative treatment plans with clients to help them accomplish activities, goals and objectives. Implement the plan through the provision of services, referrals and coordination with family and other community service providers.</li><li>
Assist clients to find appropriate housing at local shelters, board and care homes, transitional housing and independent living placements in the community.</li><li>
Assist clients in applying for entitlements and managing entitlements.</li><li>

Bilingual Case Manager

POSITION OVERVIEW:
The Case Manager in the Working Wellness Program works collaboratively with our Employment Specialist, Peer Support Specialist, and Clinician to provide case management, mental health services, and employment support services based on the IPS model. The Case Manager is responsible for supporting intakes, assessments, employment services, and ensuring effective client engagement from referral to discharge. This is an action-oriented, community-based program that aims to support clients to find meaningful employment that speaks to their interests and skills, while also addressing mental health needs so that clients can thrive.

Perform regular assessments of clients' needs in areas such as: housing, counseling, coordination of services, social relationships, family relationships, job skills, symptom management, navigating entitlement programs, budgeting, and other related areas.</li><li>
Provide case management duties for clients and connect them to various resources as necessary, including, but not limited to, assisting clients in finding housing, mental health services, resources to address food insecurity, childcare, and other supports. Support the team in meaningfully engaging with all clients a minimum of 2 times a month.</li><li>
Provide employment services to clients based on the IPS model, including conducting employer contacts with or on behalf of clients.</li><li>
Provide services in the field at least 50% of the time, including at various institutional settings and locations in the community, as necessary.</li><li>

QUALIFICATIONS &amp; EXPERIENCE:<ul><li>
BA degree in the field of psychology, counseling, or social work and two years of experience in the mental health field or four years of experience in the mental health field, or similar education/experience.</li><li>
Bilingual Spanish/English preferred.</li><li>
Experience working with Severe Mental Illness (SMI) populations and principles of case management, individual rehabilitation, family supports, supported services (housing, employment, education), and crisis interventions.</li><li>
Demonstrate a capacity and commitment to work in cooperation with a diverse team of professionals in regards to race; class; gender; sexuality; disability; age; immigration/citizenship status; religion/spirituality; and other social statuses.</li><li>
Ability to organize and prioritize duties as well as multi-task in short periods of time in regards to shifting the aforementioned roles throughout the day (i.e. community outreach, client appointments, writing case notes, IHOT meetings, consultation.)</li><li>
Demonstrate a competency of writing skills effective enough to communicate client services rendered as well as coordinate casework with IHOT staff.</li><li>
Demonstrate a capacity to collaborate in the context of team work, share casework, and communicate effectively so that program operations are executed with minimal challenges.</li><li>
Demonstrate a commitment to be teachable as well as receive and integrate feedback into work performance within an effective manner of time.</li><li>
Experience in non-profit and community-based organizations preferred.</li><li>
Demonstrates and models excellent &ldquo;customer service&rdquo; orientation and ability to work with diverse populations.</li><li>
Exercises extreme discretion with the ability to hold, balance, and transmit public and confidential information needed by various parties.</li><li>
Proficient in MS Office and Google Workspace products.</li><li>
