Taconic Rehabilitation &amp; Nursing at Hopewell is looking to add a Case Manager/ Administrative Assistant to its team. In this role, you'll provide clerical support to facility leadership, coordinate office activities, prepare reports and correspondence, assist with insurance authorization clinical reviews and Notice of Medicare Non-Coverage (NOMNC) correspondence, and help ensure the office operates efficiently while delivering excellent service to residents, families, and staff.

RESPONSIBILITIES:
<ul>
<li>Provide administrative support to the Administrator, Human Resources Director, and department managers</li>
<li>Manage calendars, meetings, correspondence, and administrative files</li>
<li>Prepare reports, letters, calendars, flyers, and other facility documents</li>
<li>Record and distribute meeting minutes</li>
<li>Complete insurance authorization clinical reviews and Notice of Medicare Non-Coverage (NOMNC) correspondence</li>
<li>Maintain resident and facility records, policy manuals, and other administrative documentation</li>
<li>Serve as backup receptionist as needed</li>
<li>Perform additional duties as assigned</li>
</ul>
REQUIREMENTS:
<ul>
<li>Minimum 18 years of age</li>
<li>Minimum of 1 year of clerical experience required</li>
<li>Long-term care experience preferred</li>
<li>Proficiency in Microsoft Office programs including Word and Excel</li>
<li>Strong organizational, written, and verbal communication skills</li>
<li>Ability to multitask in a fast-paced environment</li>
<li>Ability to file paperwork, answer and transfer calls, and follow written and verbal instructions</li>
<li>Ability to lift up to 20 pounds</li>
<li>Ability to maintain professionalism, kindness, and attention to detail</li>

Case Manager

<div>Order Entry LPN / Pharmacy Technician

Rate of Pay: $26.00 - $27.50/hr., depending on experience. (An additional $1.50/hour shift differential for hours worked between 3-11pm.)

Shift: Full-Time, Evening shift OFFICE POSITION 
Monday-Friday 3pm-11pm shift OR 
Four 10-hour week day 12:30 pm-11 pm shift 
Office Location: Hamburg or Dunkirk

Job Description: A Order Entry LPN/Pharmacy Technician works collaboratively with the pharmacy, medical providers, facility nurses, and a centralized team to enter/audit computerized physician orders within patients&rsquo; electronic health records. The candidate should have up-to-date knowledge of medication protocols. The training program is provided.

Requirements:

<li>Minimum 18 years of age</li>
<li>Medication knowledge/background required</li>
<li>Must be a Licensed Practical Nurse or a Pharmacy Technician</li>
<li>Proficiency in Point Click Care helpful</li>
<li>Strong computer, written, and verbal skills</li>
<li>Demonstrated ability to be self-directed with excellent organizational, analytical, writing, and interpersonal skills</li>
