Job Title: Designated Coordinator / Supervising ProfessionalJob OverviewA local non-profit in-home care services agency is seeking a qualified and compassionate professional to serve as a Designated Coordinator/Supervising Professional to oversee both 245D Support Services and PCA/CFSS services in the metro and surrounding areas. This role is responsible for coordinating client care, ensuring compliance with state regulations, supervising staff, and promoting person-centered service delivery.Key Responsibilities<ul><li>Conduct screenings and intakes for new clients</li><li>Develop, implement, and oversee person-centered support plans, including:</li><li>Support Plan Addendum (SPA)</li><li>Self-Management Assessment (SMA)</li><li>Individual Abuse Prevention Plan (IAPP)</li><li>Develop and manage individualized care plans and monthly service plans</li><li>Ensure compliance with Minnesota 245D licensing rules and regulations</li><li>Perform quarterly, semi-annual, and annual client reviews as required</li><li>Coordinate and oversee delivery of PCA/CFSS services</li><li>Monitor client health and safety; document changes and request reassessments as needed</li><li>Recruit, train, orient, and evaluate Direct Support Professionals (DSPs)</li><li>Provide supervision and support to assigned staff and caseload</li><li>Maintain accurate documentation and ensure high-quality service delivery</li></ul>Qualifications<ul><li>One of the following education/experience combinations:</li><li>Bachelor&rsquo;s degree in a human services-related field and at least 1 year of full-time direct care experience</li><li>Associate degree in a human services-related field and at least 2 years of full-time direct care experience</li><li>Registered nurse</li><li>Licensed social worker</li><li>Mental health professional</li><li>Designated Coordinator for intensive support services preferred or Qualified Professional experience preferred, but not required</li><li>Strong organizational and time management skills</li><li>Ability to work independently and manage multiple responsibilities</li></ul>Additional Requirements<ul><li>Successful completion of a background study conducted by the Minnesota Department of Human Services</li><li>Valid driver&rsquo;s license, clean driving record, and reliable transportation</li><li>Ability to travel independently throughout the 7-county metro and surrounding areas</li><li>Bilingual in Russian preferred, but not required</li></ul>Job TypeFull-time (flexibility may be available based on caseload needs)Benefits<ul><li>Health insurance</li><li>Dental insurance</li><li>Life insurance</li><li>Vision insurance</li><li>Paid time off</li><li>Opportunities for professional growth</li></ul>Work LocationIn person (metro area travel required)Pay: $60,000.00 - $70,000.00 per yearWork Location: In person
