Job Title: Designated Coordinator / Supervising Professional
Job Overview
A local non-profit in-home care services agency is seeking a qualified and compassionate professional to serve as a Designated Coordinator/Supervising Professional to oversee both 245D Support Services and PCA/CFSS services in the metro and surrounding areas. This role is responsible for coordinating client care, ensuring compliance with state regulations, supervising staff, and promoting person-centered service delivery.
Key Responsibilities
- Conduct screenings and intakes for new clients
- Develop, implement, and oversee person-centered support plans, including:
- Support Plan Addendum (SPA)
- Self-Management Assessment (SMA)
- Individual Abuse Prevention Plan (IAPP)
- Develop and manage individualized care plans and monthly service plans
- Ensure compliance with Minnesota 245D licensing rules and regulations
- Perform quarterly, semi-annual, and annual client reviews as required
- Coordinate and oversee delivery of PCA/CFSS services
- Monitor client health and safety; document changes and request reassessments as needed
- Recruit, train, orient, and evaluate Direct Support Professionals (DSPs)
- Provide supervision and support to assigned staff and caseload
- Maintain accurate documentation and ensure high-quality service delivery
Qualifications
- One of the following education/experience combinations:
- Bachelor’s degree in a human services-related field and at least 1 year of full-time direct care experience
- Associate degree in a human services-related field and at least 2 years of full-time direct care experience
- Registered nurse
- Licensed social worker
- Mental health professional
- Designated Coordinator for intensive support services preferred or Qualified Professional experience preferred, but not required
- Strong organizational and time management skills
- Ability to work independently and manage multiple responsibilities
Additional Requirements
- Successful completion of a background study conducted by the Minnesota Department of Human Services
- Valid driver’s license, clean driving record, and reliable transportation
- Ability to travel independently throughout the 7-county metro and surrounding areas
- Bilingual in Russian preferred, but not required
