You will play a key role in helping seniors enroll in the program, access care, and use remote monitoring technology.If you enjoy working in the community and want to make a real impact, this role is for you.Key ResponsibilitiesCommunity Outreach &amp; Enrollment<ul><li>Assist residents with program eligibility screening</li><li>Support enrollment and onboarding</li><li>Educate participants about program benefits and services</li><li>Engage with seniors in a respectful, patient-centered manner</li></ul>Remote Monitoring Support<ul><li>Assist with setup and installation of devices (blood pressure cuffs, tablets, etc.)</li><li>Teach participants how to use equipment</li><li>Troubleshoot basic issues and report concerns</li></ul>Participant Support<ul><li>Provide ongoing support and follow-up</li><li>Help coordinate appointments and communication with care teams</li><li>Identify barriers to care and report to program staff</li></ul>Field Work<ul><li>Travel to multiple locations across Baltimore City</li><li>Work in homes, community centers, and partner sites</li><li>Represent the program professionally in the community</li></ul>Requirements<ul><li>Active Maryland CNA license</li><li>Must pass CJIS background check</li><li>Strong communication and interpersonal skills</li><li>Comfortable working in community/home environments</li><li>Ability to use basic technology (smartphones, tablets, apps)</li></ul>Preferred Qualifications<ul><li>Experience with seniors or community health programs</li><li>Background in outreach or patient engagement</li><li>Bilingual candidates encouraged to apply</li></ul>TransportationReliable transportation strongly recommended (This role requires travel between multiple sites in Baltimore City)Why Join Us<ul><li>Make a direct impact in your community</li><li>Work with a mission-driven healthcare program</li><li>Gain experience in telehealth and remote patient monitoring</li><li>Opportunity to grow with an expanding program</li></ul>Apply TodayJoin MTCI Healthcare Services and help improve access to care for Baltimore seniors through the Soteria 360 Senior Thrive Program.Pay: $22.00 - $25.00 per hourWork Location: In person
