• Training: 3 days/week, 8-hour shifts
• Full Program: 5 days/week, 8-hour shifts
• Day shifts; flexible scheduling based on community needs

Job Description

MTCI Healthcare Services is hiring a Certified Nursing Assistant (CNA) to support the Soteria 360 Senior Thrive Program, a community-based initiative delivering preventive and telehealth services to seniors across Baltimore City.

This is a mobile, field-based role where you will work directly with residents in their homes and community settings. You will play a key role in helping seniors enroll in the program, access care, and use remote monitoring technology.

Key Responsibilities

Community Outreach & Enrollment

• Assist residents with program eligibility screening
• Support enrollment and onboarding
• Educate participants about program benefits and services
• Engage with seniors in a respectful, patient-centered manner

Remote Monitoring Support

• Assist with setup and installation of devices (blood pressure cuffs, tablets, etc.)
• Teach participants how to use equipment
• Troubleshoot basic issues and report concerns

Participant Support

• Provide ongoing support and follow-up
• Help coordinate appointments and communication with care teams
• Identify barriers to care and report to program staff

Field Work

• Travel to multiple locations across Baltimore City
• Work in homes, community centers, and partner sites
• Represent the program professionally in the community

Requirements

• Active Maryland CNA license
• Must pass CJIS background check
• Strong communication and interpersonal skills
• Comfortable working in community/home environments
• Ability to use basic technology (smartphones, tablets, apps)

Preferred Qualifications

• Experience with seniors or community health programs
• Background in outreach or patient engagement
• Bilingual candidates encouraged to apply

Transportation

Reliable transportation strongly recommended
(This role requires travel between multiple sites in Baltimore City)
