We are currently seeking a dedicated and compassionate ECM Case Manager to join our team.Job Description:As an ECM (Enhanced Case Manager), you will play a crucial role in helping our clients manage their health and well-being. You will work closely with clients to develop and implement individualized care plans, ensuring they receive the necessary services and support. Your primary responsibilities will include:** This is an in-person position. Remote or hybrid work is not available.**Key Responsibilities:<ul><li>Outreach and Engagement:</li><li>Locate, contact, and engage members identified as candidates for ECM using various strategies, including in-person meetings and digital communications.</li><li>Conduct outreach and engagement in a culturally and linguistically appropriate manner.</li><li>Document all outreach and engagement attempts and share relevant information with health plans.</li><li>Comprehensive Assessment and Care Management Planning:</li><li>Conduct comprehensive assessments to identify clinical and non-clinical needs.</li><li>Develop and maintain individualized, person-centered care plans in collaboration with members and their support systems.</li><li>Regularly reassess and update care plans based on changes in members&rsquo; needs.</li><li>Enhanced Coordination of Care:</li><li>Assist members in navigating health, behavioral health, and social service systems.</li><li>Maintain regular communication with members&rsquo; care teams and coordinate appointments and transportation as needed.</li><li>Monitor referrals and ensure continuous, integrated care.</li><li>Health Promotion:</li><li>Provide health education and coaching to members and their support systems.</li><li>Support members in making healthy choices and managing their health conditions.</li><li>Link members to resources for smoking cessation, chronic condition management, and other health-related services.</li><li>Comprehensive Transitional Care:</li><li>Facilitate smooth transitions between different care settings, such as hospitals and nursing facilities.</li><li>Develop and update transition plans and coordinate post-discharge support services.</li><li>Educate members on self-management and medication management.</li><li>Member and Family Supports:</li><li>Document and communicate with members' families and support systems as needed.</li><li>Provide education and resources to support members' health goals and treatment adherence.</li><li>Ensure members and their families have access to and understand their care plans.</li><li>Coordination of and Referral to Community and Social Supports:</li><li>Identify members&rsquo; needs for community and social services, such as food assistance or housing support.</li><li>Make appropriate referrals and follow up to ensure members receive necessary services.</li></ul>Qualifications:<ul><li>Background in social work, psychology, or a related field is helpful but not required.</li><li>Experience in the fields of social work or case management is a plus.</li><li>Strong understanding of medical, behavioral, and social determinants of health.</li><li>Ability to use or learn electronic health records.</li><li>Strong determination to learn and adapt in a dynamic environment.</li><li>Excellent communication and interpersonal skills.</li><li>Ability to work independently and as part of a multidisciplinary team.</li><li>Valid driver's license and reliable transportation.</li></ul>Skills:<ul><li>Strong organizational and time-management skills.</li><li>Empathy and compassion for working with diverse populations.</li><li>Ability to develop and implement effective care plans.</li><li>Knowledge of community resources and social services.</li><li>Problem-solving and critical-thinking abilities.</li><li>Ability to educate and support clients and their families.</li></ul>Benefits:<ul><li>We provide medical, dental, vision insurance, life insurance</li><li>40 hours of vacation time (usable after the first 6 months)</li><li>401k plan (after 1 year)</li><li>Job Type: Full-time</li><li>Pay: $24.00 - $26.00 per hour</li></ul>Schedule:<ul><li>Monday to Friday</li></ul>Work Location: In personJob Type: Full-timePay: $24.00 - $26.00 per hourExpected hours: 40 per weekBenefits:<ul><li>Dental insurance</li><li>Health insurance</li><li>Paid time off</li><li>Vision insurance</li></ul>Ability to Commute:<ul><li>Chico, CA 95927 (Preferred)</li></ul>Ability to Relocate:<ul><li>Chico, CA 95927: Relocate before starting work (Preferred)</li></ul>Work Location: In person
