assessing treatment needs, conducting orientations:Key Responsibilities<ul><li>Assessment &amp; Planning: Conduct comprehensive assessments of behavioral health needs, strengths, and barriers to create personalized treatment or recovery plans.</li><li>Care Coordination: Link clients to community resources, including, but not limited to, mental health services, substance use treatment (rehab), housing, and employment support.</li><li>Monitoring &amp; Advocacy: Track client progress, adjust care plans, and advocate for clients to ensure timely access to services.</li><li>Support &amp; Education: Provide psychoeducation, life-skills training, and supportive counseling to clients and families.</li><li>Documentation: Maintain accurate, timely, and secure client records in compliance with regulatory standards.</li><li>Crisis Management: Offer immediate support during acute distress or emergencies.</li></ul>Required Qualifications &amp; Skills<ul><li>Education: Bachelor&rsquo;s degree in Social Work, Psychology, Counseling, or a related field.</li><li>Experience: Previous experience in case management, mental health, or substance abuse treatment is typically required.</li><li>Skills: Strong communication, empathy, problem-solving, and organization skills are essential.</li><li>Knowledge: Understanding of evidence-based practices, trauma-informed care, and community resources.
