Key Responsibilities

  • Assessment & Planning: Conduct comprehensive assessments of behavioral health needs, strengths, and barriers to create personalized treatment or recovery plans.
  • Care Coordination: Link clients to community resources, including, but not limited to, mental health services, substance use treatment (rehab), housing, and employment support.
  • Monitoring & Advocacy: Track client progress, adjust care plans, and advocate for clients to ensure timely access to services.
  • Support & Education: Provide psychoeducation, life-skills training, and supportive counseling to clients and families.
  • Documentation: Maintain accurate, timely, and secure client records in compliance with regulatory standards.
  • Crisis Management: Offer immediate support during acute distress or emergencies.

Required Qualifications & Skills

  • Education: Bachelor’s degree in Social Work, Psychology, Counseling, or a related field.
  • Experience: Previous experience in case management, mental health, or substance abuse treatment is typically required.
  • Skills: Strong communication, empathy, problem-solving, and organization skills are essential.

Work Environment
Case managers assist in diverse settings, outpatient clinics, community health centers, and residential treatment facilities, often conducting visits in the community or client homes

Job Type: Full-time

Pay: From $18.00 per hour

Expected hours: 32 per week
