This position is ideal for a clinician who enjoys independent practice, patient interaction, and making a direct impact on healing outcomes.Position Details<ul><li>Schedule: Part-time (approximately 10&ndash;20 hours per week)</li><li>Location: Maryland</li><li>Travel: Required between patient homes and facilities</li><li>Preferred Availability: Ability to work a full 8-hour day on Monday, Thursday, or Sunday</li><li>Primary Service Area: Philadelphia, Montgomery, and Bucks</li></ul>Candidates should live within or near these counties to allow efficient travel to patients.Responsibilities<ul><li>Conduct comprehensive patient assessments focusing on wound evaluation, including physical examinations and vital signs monitoring.</li><li>Develop and implement individualized wound care plans utilizing evidence-based practices and advanced treatments such as Botulinum toxin therapy when appropriate.</li><li>Perform specialized procedures including debridement, and dressing changes.</li><li>Manage complex cases involving diabetic foot ulcers, pressure injuries, surgical wounds, and infectious wounds .</li><li>Collaborate with other healthcare professionals to coordinate holistic patient care.</li><li>Utilize electronic health record (EHR) systems like Epic or eClinicalWorks for accurate documentation of patient encounters, treatment plans, and progress notes.</li><li>Educate patients and their families on wound prevention strategies, proper dressing techniques, nutrition, and lifestyle modifications to promote healing.</li></ul>Qualifications<ul><li>Licensed Nurse Practitioner</li><li>Minimum 2 years of wound care and/or home health experience</li><li>Strong knowledge of wound evaluation, treatment planning, and healing progression</li><li>Experience managing chronic or complex wounds</li><li>Comfortable performing debridement and advanced wound care procedures</li><li>Experience working with older adult populations</li><li>Ability to work independently while traveling between patient locations</li><li>Strong communication skills for patient and caregiver education</li><li>Familiarity with EMR systems and clinical documentation</li><li>Understanding of HIPAA and patient privacy standards</li></ul>Additional Requirements<ul><li>Must live in or near Lehigh, Northampton, or Carbon County</li><li>Reliable transportation for travel to patient homes and facilities</li><li>Stable work history with consistent employment</li><li>Ability to provide professional references upon request</li></ul>To help us review your application more quickly, please complete the short application form below.
