Overview

Optimal Wound Care is seeking a Wound Care Nurse Practitioner to provide specialized wound management for patients in their homes and assisted living communities across the Lehigh Valley region.

Our mobile wound care practice focuses on treating chronic and complex wounds, helping patients receive advanced care without needing to travel to a clinic.

In this role, you will assess, diagnose, and manage wounds while working collaboratively with patients, families, and healthcare partners. This position is ideal for a clinician who enjoys independent practice, patient interaction, and making a direct impact on healing outcomes.

Position Details

  • Schedule: Part-time (approximately 10–20 hours per week)
  • Location: Maryland
  • Travel: Required between patient homes and facilities
  • Preferred Availability: Ability to work a full 8-hour day on Monday, Thursday, or Sunday
  • Primary Service Area: Philadelphia, Montgomery, and Bucks

Candidates should live within or near these counties to allow efficient travel to patients.

Responsibilities

  • Conduct comprehensive patient assessments focusing on wound evaluation, including physical examinations and vital signs monitoring.
  • Develop and implement individualized wound care plans utilizing evidence-based practices and advanced treatments such as Botulinum toxin therapy when appropriate.
  • Perform specialized procedures including debridement, and dressing changes.
  • Manage complex cases involving diabetic foot ulcers, pressure injuries, surgical wounds, and infectious wounds .
  • Collaborate with other healthcare professionals to coordinate holistic patient care.
  • Utilize electronic health record (EHR) systems like Epic or eClinicalWorks for accurate documentation of patient encounters, treatment plans, and progress notes.
  • Educate patients and their families on wound prevention strategies, proper dressing techniques, nutrition, and lifestyle modifications to promote healing.

Qualifications

  • Licensed Nurse Practitioner
  • Minimum 2 years of wound care and/or home health experience
  • Strong knowledge of wound evaluation, treatment planning, and healing progression
  • Experience managing chronic or complex wounds
  • Comfortable performing debridement and advanced wound care procedures
  • Experience working with older adult populations
  • Ability to work independently while traveling between patient locations
  • Strong communication skills for patient and caregiver education
  • Familiarity with EMR systems and clinical documentation
  • Understanding of HIPAA and patient privacy standards

Additional Requirements

  • Must live in or near Lehigh, Northampton, or Carbon County
  • Reliable transportation for travel to patient homes and facilities
  • Stable work history with consistent employment
  • Ability to provide professional references upon request

Pay: $70.00 - $120.00 per hour
