Admissions Coordinator Responsibilities:<ul><li>
Coordinate and facilitate the admission process for new patients, ensuring efficient and effective communication with patients, families, and healthcare providers</li><li>
Conduct admissions assessments to gather necessary information and complete required paperwork</li><li>
Build relationships with healthcare professionals, insurance companies, and community organizations to promote our services and referring patients to our center</li><li>
Utilize electronic medical records to document and track patient information</li><li>
Collaborate with the interdisciplinary team to ensure a comprehensive understanding of patients' needs and develop individualized care plans</li><li>
Provide exceptional customer service to patients, families, and caregivers to ensure a positive experience during the admission process</li></ul>
Admissions Coordinator Requirements:<ul><li>
Min 2 years of experience in a SNF, in the Admissions Department</li><li>
Education in a healthcare-related field preferred</li><li>
Must have knowledge/experience with Medicare, Medicaid, Insurances and be able to complete verifications.</li><li>
Must have strong PR skills, be self-directed, able to manage a busy environment.</li><li>
Requires some weekends</li></ul>
