Page Rehabilitation is seeking a dynamic and organized Admissions Coordinator to join our team! As a key member of our intake process, you will be responsible for ensuring a smooth transition for new patients and families. Admissions Coordinator Responsibilities:

  • Coordinate and facilitate the admission process for new patients, ensuring efficient and effective communication with patients, families, and healthcare providers

  • Conduct admissions assessments to gather necessary information and complete required paperwork

  • Build relationships with healthcare professionals, insurance companies, and community organizations to promote our services and referring patients to our center

  • Utilize electronic medical records to document and track patient information

  • Collaborate with the interdisciplinary team to ensure a comprehensive understanding of patients' needs and develop individualized care plans

  • Provide exceptional customer service to patients, families, and caregivers to ensure a positive experience during the admission process

Admissions Coordinator Requirements:

  • Min 2 years of experience in a SNF, in the Admissions Department

  • Education in a healthcare-related field preferred

  • Must have knowledge/experience with Medicare, Medicaid, Insurances and be able to complete verifications.

  • Must have strong PR skills, be self-directed, able to manage a busy environment.

  • Requires some weekends
