Precision Healthcare Specialists is seeking a compassionate, organized, and patient-focused <b>Nurse Navigator</b> to support patients throughout their healthcare journey. The Nurse Navigator serves as a key point of contact for patients, families, providers, and interdisciplinary care teams by coordinating care, providing education, assisting with referrals, and helping remove barriers that may impact access to timely treatment.</p><p>

Essential Duties and Responsibilities</b></h2><ul><li>
Serve as a primary point of contact for patients and their families throughout the continuum of care.</li><li>
Coordinate and facilitate timely patient care, including appointments, diagnostic testing, treatment, referrals, and follow-up services.</li><li>
Assess patients&rsquo; clinical, educational, emotional, financial, and psychosocial needs.</li><li>
Provide patients and families with clear education regarding diagnoses, treatment plans, procedures, medications, and available resources.</li><li>
Collaborate closely with physicians, advanced practice providers, medical assistants, clinical staff, specialists, and other members of the healthcare team.</li><li>
Assist patients with navigating complex healthcare systems and understanding recommended care plans.</li><li>
Identify barriers that may prevent patients from receiving timely or appropriate care and assist in developing solutions.</li><li>
Coordinate referrals to specialists, community resources, support services, financial assistance programs, and other appropriate services.</li><li>
Monitor patient progress and follow up to ensure recommended appointments, testing, treatments, and care plans are completed.</li><li>
Review patient records and clinical information to support care coordination and continuity of care.</li><li>
Communicate relevant patient concerns, changes in condition, and care needs to the appropriate provider or clinical team.</li><li>
Assist with care transitions and coordination between healthcare providers, facilities, and community resources.</li><li>
Maintain accurate, complete, and timely documentation in the electronic health record (EHR).</li><li>
Participate in multidisciplinary meetings, case reviews, care planning discussions, and quality-improvement initiatives.</li><li>
Maintain patient confidentiality and comply with all HIPAA, organizational, state, and federal requirements.</li><li>
Provide compassionate support and advocacy while promoting patient engagement and informed decision-making.</li><li>
Maintain current knowledge of clinical guidelines, treatment options, healthcare resources, and applicable standards of care.</li><li>
Perform other duties and responsibilities as assigned.</li></ul>
Requirements:<br>
<p></p><ul><li>
Current and unrestricted <b>Registered Nurse (RN) license</b> in the State of Florida.</li><li>
Graduate of an accredited nursing program.</li><li>
Minimum of <b>[2&ndash;3] years of clinical nursing experience</b>, preferably in a specialty practice, outpatient setting, care coordination, case management, oncology, or another complex-care environment.</li><li>
Strong knowledge of clinical care coordination and patient education principles.</li><li>
Excellent verbal and written communication skills.</li><li>
Strong organizational, critical-thinking, and problem-solving abilities.</li><li>
Ability to manage multiple patients, responsibilities, and competing priorities effectively.</li><li>
Ability to work independently while collaborating closely with an interdisciplinary healthcare team.</li><li>
Proficiency with electronic health records (EHRs) and standard computer applications.</li><li>
Strong attention to detail and commitment to providing high-quality patient care.</li></ul><h2 class="jobSectionHeader"><b>
Preferred Qualifications</b></h2><ul><li>
Bachelor of Science in Nursing (BSN).</li><li>
Previous experience as a Nurse Navigator, Care Coordinator, Case Manager, or Clinical Care Coordinator.</li><li>
Experience working with patients who have complex, chronic, or specialty healthcare needs.</li><li>
Certification in nursing navigation, case management, care coordination, or a related specialty.</li><li>
Experience in an outpatient or specialty medical practice.</li><li>
Bilingual abilities are a plus.</li></ul><h2 class="jobSectionHeader"><b>

This position primarily works in a professional outpatient healthcare setting. The CMA will play a vital role in supporting physicians and advanced practice providers by delivering exceptional patient care, assisting with clinical procedures, and ensuring efficient daily operations within a fast-paced specialty practice. The ideal candidate is highly organized, patient-focused, and experienced in providing clinical support in a specialty healthcare setting.</p><h2 class="jobSectionHeader"><b>
Essential Duties and Responsibilities</b></h2><ul><li>
Room patients and prepare them for examinations and procedures</li><li>
Obtain and accurately document vital signs, medical histories, medications, allergies, and chief complaints</li><li>
Assist physicians and providers during examinations, biopsies, minor procedures, and treatments</li><li>
Administer injections, medications, and perform clinical procedures within scope of practice</li><li>
Collect and prepare specimens for laboratory testing</li><li>
Perform EKGs, point-of-care testing, and other diagnostic procedures as directed</li><li>
Educate patients regarding treatment plans, medications, pre-operative and post-operative instructions</li><li>
Coordinate referrals, imaging studies, laboratory testing, and specialty consultations</li><li>
Process prescription refill requests and prior authorizations</li><li>
Maintain accurate and timely documentation within the electronic medical record (EMR)</li><li>
Prepare and maintain examination rooms, equipment, and clinical supplies</li><li>
Follow all HIPAA, OSHA, infection control, and company policies and procedures</li><li>
Provide exceptional customer service while maintaining patient confidentiality and professionalism</li><li>
Collaborate with providers, clinical staff, and administrative teams to ensure continuity of care</li><li>

Requirements:<br>

High School Diploma or equivalent required</li><li>
Current Certified Medical Assistant (CMA), Registered Medical Assistant (RMA), or equivalent certification required</li><li>
Minimum of 1-2 years of Medical Assistant experience in a specialty practice preferred</li><li>
Experience in ENT, Head &amp; Neck Surgery, Oncology, General Surgery, or related specialty preferred</li><li>
Knowledge of medical terminology, clinical procedures, and patient care practices</li><li>
Proficiency with EMR systems and Microsoft Office applications</li><li>
Excellent communication, organizational, and multitasking skills</li><li>
Ability to work efficiently in a fast-paced clinical environment</li><li>
