Your energetic approach and commitment to excellence will drive positive outcomes and foster trusting relationships with patients and their families.Responsibilities<ul><li>Conduct thorough patient assessments, including vital signs, physical examinations, and health histories in a home setting.</li><li>Develop and implement individualized care plans tailored to each patient's medical needs and cultural background.</li><li>Administer medications, injections, IV infusions, and perform wound care such as suturing and ostomy management with precision and aseptic technique.</li><li>Provide education on disease management, medication adherence, nutrition, and self-care strategies in both English and Spanish to empower patients.</li><li>Coordinate with physicians, therapists, and other healthcare professionals to ensure seamless continuity of care across settings such as assisted living, nursing homes, hospitals, or outpatient clinics.</li><li>documentation of patient care activities while maintaining HIPAA compliance.</li><li>Respond promptly to urgent situations through triage protocols, acute pain management techniques, or emergency interventions as needed.</li></ul>Requirements<ul><li>Valid RN or LPN license with current state certification; bilingual fluency in English and Spanish is essential.</li><li>Experience in home health care or medical-surgical nursing strongly preferred; familiarity with long-term care, hospice &amp; palliative medicine, or geriatrics is a plus.</li><li>Critical care experience including ICU or trauma center background highly desirable; experience with IV insertion, catheterization, laboratory specimen collection/processing is beneficial.</li><li>Strong understanding of medical terminology, physiology knowledge, infection control practices, and aseptic techniques.</li><li>Ability to work independently in patients&rsquo; homes while managing multiple cases efficiently; excellent communication skills in both Spanish and English are required.</li><li>Valid driver&rsquo;s license with reliable transportation; willingness to travel within assigned service areas.</li><li>Join our team of dedicated professionals committed to providing exceptional home health care.
