Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and health histories in a home setting.
- Develop and implement individualized care plans tailored to each patient's medical needs and cultural background.
- Administer medications, injections, IV infusions, and perform wound care such as suturing and ostomy management with precision and aseptic technique.
- Provide education on disease management, medication adherence, nutrition, and self-care strategies in both English and Spanish to empower patients.
- Coordinate with physicians, therapists, and other healthcare professionals to ensure seamless continuity of care across settings such as assisted living, nursing homes, hospitals, or outpatient clinics.
- documentation of patient care activities while maintaining HIPAA compliance.
- Respond promptly to urgent situations through triage protocols, acute pain management techniques, or emergency interventions as needed.
Requirements
- Valid RN or LPN license with current state certification; bilingual fluency in English and Spanish is essential.
- Experience in home health care or medical-surgical nursing strongly preferred; familiarity with long-term care, hospice & palliative medicine, or geriatrics is a plus.
- Critical care experience including ICU or trauma center background highly desirable; experience with IV insertion, catheterization, laboratory specimen collection/processing is beneficial.
- Strong understanding of medical terminology, physiology knowledge, infection control practices, and aseptic techniques.
- Ability to work independently in patients’ homes while managing multiple cases efficiently; excellent communication skills in both Spanish and English are required.
- Valid driver’s license with reliable transportation; willingness to travel within assigned service areas.

Pay: $30.00 - $45.00 per hour
