RNS Private Duty Care

<div>Job Title: RN Case Manager &ndash; Home Care
Location: Leawood, KS &ndash; Hybrid (remote/hybrid)
Schedule: Part-time, 15-20 hours per week, with weekend on-call rotation every 5 weeks
Compensation: Hourly, $60/hr: on-site (in person) case management work
$35/hr: case managing work via telephone or text

Responsibilities:<ul><li>
Assess clients and their homes to determine care needs</li><li>
Train, oversee, and support caregivers in providing high-quality care</li><li>
Update and create individualized care plans</li><li>
Provide patient and family education on care routines</li><li>
Serve as a clinical resource for caregivers</li><li>
Collaborate with office staff and attend occasional in-office meetings/trainings</li></ul>
Qualifications:<ul><li>
RN license required</li><li>
Current CPR certification or completion within 90 days of hire</li><li>
Current TB test or completion within 90 days of hire</li><li>
Minimum 1 year as a licensed nurse preferred</li><li>
Strong skills in care planning, patient/family education, and leadership</li><li>
Licensed driver with a clean driving record and reliable vehicle</li><li>
Cellphone with internet and texting capabilities; daily access to email</li><li>
Ability to meet physical and mental demands of the position</li><li>
Comply with RNS policies, HIPAA, dress code, and weekend/holiday availability</li></ul>

Schedule: Part-time, 15-20 hours per week, with weekend on-call rotation every 5 weeks
Compensation: Hourly, $50/hr for any case management activities that occur in person and on site, $28/hr for any case management activities that are handled via text or phone
