Social Worker - General DescriptionResponsible for providing direct clinical care in integrated treatment, rehabilitation, and support services to ACT recipients who experience severe mental health symptoms and may have co-occurring substance use conditions. Meets with recipients regularly in the community to assist with vocational rehabilitation and employment goals.Schedule<ul><li>Mondays-Fridays 9am-5pm</li><li>On-call rotation about every 6 weeks</li></ul>Positions Available<ul><li>Family Specialist</li><li>Employment Specialist</li></ul>Social Worker - Job Description<ul><li>Provides face-to-face treatment and support within specialty area in the community.</li><li>Develops, modifies, and implements person-centered vocational goals with ACT recipients.</li><li>Offers counseling and supports around job readiness, continuing education, and vocational training.</li><li>Identifies employment barriers and helps clients build coping strategies and life skills.</li><li>Provides trauma-informed, culturally sensitive care using evidence-based practices.</li><li>Delivers interventions such as motivational interviewing, harm reduction, relapse prevention, etc.</li><li>Collaborates with the ACT team, family members, and community resources for integrated support.</li><li>Educates team members within specialty role and provides generalist support when needed.</li><li>Participates in on-call rotation and responds to psychiatric or medical emergencies.</li><li>Attends daily team meetings, submits timely documentation, and engages in supervision.</li><li>Acts as liaison to employment services, vocational rehab programs, and job training organizations.</li><li>Advocates for clients&rsquo; rights and ensures entitlements and referrals are in place.</li></ul>Social Worker - Qualifications<ul><li>Master&rsquo;s Degree in Human Services with certification or licensure in one of the following:</li><li>Social Work</li><li>Mental Health Counseling</li><li>Vocational Rehabilitation Counseling</li><li>CASAC or equivalent specialization</li><li>3+ years experience in human services; 1+ year in vocational services preferred.</li><li>Experience with mobile, multidisciplinary teams preferred.</li><li>Strong communication, organizational, and decision-making skills.</li><li>Ability to build inclusive employment plans aligned with participants&rsquo; backgrounds and goals.</li></ul>Physical Demands &amp; Work Conditions<ul><li>Frequent travel to participant homes, job sites, and community resources.</li><li>Work hours between 8AM&ndash;8PM with on-call coverage.</li><li>Direct, in-person contact with clients required.</li><li>Ability to support clients from diverse cultural and socioeconomic backgrounds.</li></ul>EEO StatementSAIL does not discriminate on the basis of race, color, religion, sex, sexual orientation, gender identity or expressions, national origin, age, disability, veteran status, marital status, or based on an individual status in any group or class protected by applicable federal, state or local law.
