Social Worker/Family or Employment Specialist

Social Worker - General Description

Responsible for providing direct clinical care in integrated treatment, rehabilitation, and support services to ACT recipients who experience severe mental health symptoms and may have co-occurring substance use conditions. Meets with recipients regularly in the community to assist with vocational rehabilitation and employment goals.

Schedule

  • Mondays-Fridays 9am-5pm
  • On-call rotation about every 6 weeks

Social Worker - Job Description

  • Provides face-to-face treatment and support within specialty area in the community.
  • Develops, modifies, and implements person-centered vocational goals with ACT recipients.
  • Offers counseling and supports around job readiness, continuing education, and vocational training.
  • Identifies employment barriers and helps clients build coping strategies and life skills.
  • Provides trauma-informed, culturally sensitive care using evidence-based practices.
  • Delivers interventions such as motivational interviewing, harm reduction, relapse prevention, etc.
  • Collaborates with the ACT team, family members, and community resources for integrated support.
  • Educates team members within specialty role and provides generalist support when needed.
  • Participates in on-call rotation and responds to psychiatric or medical emergencies.
  • Attends daily team meetings, submits timely documentation, and engages in supervision.
  • Acts as liaison to employment services, vocational rehab programs, and job training organizations.
  • Advocates for clients’ rights and ensures entitlements and referrals are in place.

Social Worker - Qualifications

  • Master’s Degree in Human Services with certification or licensure in one of the following:
  • Social Work
  • Mental Health Counseling
  • Vocational Rehabilitation Counseling
  • CASAC or equivalent specialization
  • 3+ years experience in human services; 1+ year in vocational services preferred.
  • Experience with mobile, multidisciplinary teams preferred.
  • Strong communication, organizational, and decision-making skills.
  • Ability to build inclusive employment plans aligned with participants’ backgrounds and goals.

Physical Demands & Work Conditions

  • Frequent travel to participant homes, job sites, and community resources.
  • Work hours between 8AM–8PM with on-call coverage.
  • Direct, in-person contact with clients required.
  • Ability to support clients from diverse cultural and socioeconomic backgrounds.
