Social Work Manager
Role Summary 
Our client is seeking a compassionate and experienced Social Work Manager to lead their Social Work team and guide the delivery of high-quality social services and behavioral health programs. Population: Geriatric population
Duties<ul><li>
Lead and mentor the Social Work team, including Supervisors, Social Workers, Behavioral Health Specialists, and Assistants&mdash;providing hiring, training, supervision, and coaching.</li><li>
Oversee the quality and effectiveness of social work assessments and team participation.</li><li>
Ensure delivery of all social work interventions aligned with patient care plans and responsive to urgent needs.</li><li>
Maintain compliance across social work functions, including documentation, assessments, care planning, reviews, and patient rights and grievance processes.</li><li>
Collaborate with Behavioral Health Specialists to design and oversee support groups, individual counseling, family education, and community partnerships.</li><li>
Support patients and families through end-of-life care planning and coordination.</li><li>
Conduct home visits as needed to support patients&rsquo; ongoing care.</li></ul>
Required Qualifications<ul><li>
Master&rsquo;s degree in Social Work (MSW).</li><li>
Active state driver&rsquo;s license with clean DMV record.</li><li>
Proof of recent vehicle insurance.</li><li>
Minimum 1 year of experience working with a frail or elderly population.</li><li>
Excellent clinical, organizational, and communication skills with patients, families, and team members.</li></ul>
Desired Qualifications<ul><li>
Social work licensure (preferred).</li><li>
At least 2 years of supervisory or leadership experience in social work.

Setting: Outpatient Clinic working with the geriatric population.
Responsibilities:<ul><li>
Conduct social work assessments to identify the psychosocial needs, preferences, and goals of patients, contributing to the development of personalized care plans.</li><li>
Actively participate in team meetings to collaborate on care plan creation and patient management.</li><li>
Provide social work interventions as outlined in patient care plans, including resource arrangement, care transitions, individual and group counseling, and case management.</li><li>
Partner with primary care physicians and other team members to ensure smooth transitions of care between settings, such as hospitals, skilled nursing facilities, and home care.</li><li>
Coordinate, facilitate, and lead care conference meetings to ensure effective communication and care coordination with patients, families, caregivers, and team members.</li><li>
Offer discharge planning and support when patients disenroll from the program.</li><li>
Other duties as assigned</li></ul><ul><li>

Base Salary: $65,089 &ndash; $104,850 annually (depending on experience and qualifications)</li><li>
