Our client is seeking a dedicated and compassionate Social Worker to join their team, playing a key role in providing high-quality, comprehensive care to frail seniors. The Social Worker will work closely with a diverse team of healthcare professionals to ensure that patients receive the best possible support and services. This role is ideal for individuals who are passionate about making a positive impact in the lives of seniors and fostering a holistic approach to their care.
Setting: Outpatient Clinic working with the geriatric population.
Responsibilities:<ul><li>
Conduct social work assessments to identify the psychosocial needs, preferences, and goals of patients, contributing to the development of personalized care plans.</li><li>
Actively participate in team meetings to collaborate on care plan creation and patient management.</li><li>
Provide social work interventions as outlined in patient care plans, including resource arrangement, care transitions, individual and group counseling, and case management.</li><li>
Partner with primary care physicians and other team members to ensure smooth transitions of care between settings, such as hospitals, skilled nursing facilities, and home care.</li><li>
Coordinate, facilitate, and lead care conference meetings to ensure effective communication and care coordination with patients, families, caregivers, and team members.</li><li>
Offer discharge planning and support when patients disenroll from the program.</li><li>
Other duties as assigned</li></ul><ul><li>
Master&rsquo;s Degree in Social Work (MSW).</li><li>
A minimum of one (1) year of experience working with a frail or elderly population.</li><li>
Experience coordinating and facilitating care conference meetings.</li><li>
Perfered experience working with individuals who have behavioral health and substance abuse issues.</li></ul>

Key Responsibilities:</h3><ul><li>
Provide therapeutic and diagnostic respiratory services across a variety of clinical settings</li><li>

Operate and maintain advanced ventilatory support systems and monitoring equipment</li><li>
Assist with resuscitation, oxygen therapy, airway management, and diagnostic procedures</li><li>
Complete comprehensive patient assessments and document findings accurately</li><li>
Support patient care in both routine and emergent situations</li><li>
Participate in orientation and ongoing training to maintain high clinical standards</li><li>

Required Qualifications:</h3><ul><li>
Completion of education required to meet state licensure standards</li><li>
Active Washington Respiratory Care Practitioner License (or ability to obtain upon hire)</li><li>
Current BLS certification from the American Heart Association</li><li>
Proven ability to work collaboratively and deliver patient-centered care</li></ul><h3 class="jobSectionHeader">

Base Salary: $65,089 &ndash; $104,850 annually (depending on experience and qualifications)</li><li>
