Overview We are seeking dedicated Licensed Practical Nurses (LPNs) for both full-time and part-time positions to join our healthcare team. This role offers an excellent opportunity to provide compassionate patient care for our skilled nursing facility. The ideal candidates will possess a strong clinical background, proficiency with electronic health records, and a passion for improving patient outcomes. Experience with PCC Candidates should be committed to maintaining HIPAA compliance and delivering high-quality nursing services in a fast-paced environment.Duties<ul><li>Perform comprehensive patient assessments including vital signs, physical examinations, and medical history documentation.</li><li>Administer medications and injections accurately following physician orders while adhering to sterile techniques and aseptic procedures.</li><li>Manage catheter care, and medication pass.</li><li>Assist with diagnostic evaluations such as blood draws (phlebotomy), specimen collection, and processing.</li><li>Provide wound care</li><li>Monitor patients&rsquo; physiological status and respond promptly to changes in condition, including airway management and acute pain management.</li><li>Document all patient interactions thoroughly in EMR/EHR systems such as Athenahealth or Meditech while ensuring compliance with HIPAA standards.</li><li>Coordinate discharge planning, case management, and patient education tailored to individual needs including dementia care, Alzheimer&rsquo;s care, or pediatric assessments.</li><li>Collaborate with multidisciplinary teams in settings like ICU, emergency room, outpatient clinics, or home &amp; community care environments.</li><li>Support infection control protocols and maintain sterile processing standards to ensure patient safety.</li></ul>Experience<ul><li>Proven experience in diverse healthcare environments such as long term care and rehab.</li><li>Skilled in managing complex cases including infectious disease care, pain management, behavioral health cases, and post-acute care scenarios.</li><li>Prior work with special populations such as geriatrics (memory care)</li><li>Proficiency in medical terminology and physiology knowledge essential for comprehensive patient assessment.</li><li>Experience administering injections (including botulinum toxin treatments), performing venipuncture or catheterization procedures.</li><li>This position offers an engaging environment where you can make a meaningful difference in patients&rsquo; lives while advancing your nursing career through diverse clinical experiences across multiple healthcare settings.</li></ul>All candidates must be licensed in the state of Illinois with a professional license that is in good standing.Pay: $30.00 - $35.00 per hourExpected hours: 16.0 &ndash; 40.0 per weekBenefits:<ul><li>401(k)</li><li>Dental insurance</li><li>Health insurance</li><li>Health savings account</li><li>Life insurance</li><li>Paid time off</li></ul>Work Location: In person
