Overview
We are seeking dedicated Licensed Practical Nurses (LPNs) for both full-time and part-time positions to join our healthcare team. This role offers an excellent opportunity to provide compassionate patient care for our skilled nursing facility. The ideal candidates will possess a strong clinical background, proficiency with electronic health records, and a passion for improving patient outcomes. Duties
- Perform comprehensive patient assessments including vital signs, physical examinations, and medical history documentation.
- Administer medications and injections accurately following physician orders while adhering to sterile techniques and aseptic procedures.
- Manage catheter care, and medication pass.
- Assist with diagnostic evaluations such as blood draws (phlebotomy), specimen collection, and processing.
- Provide wound care
- Monitor patients’ physiological status and respond promptly to changes in condition, including airway management and acute pain management.
- Document all patient interactions thoroughly in EMR/EHR systems such as Athenahealth or Meditech while ensuring compliance with HIPAA standards.
- Coordinate discharge planning, case management, and patient education tailored to individual needs including dementia care, Alzheimer’s care, or pediatric assessments.
- Collaborate with multidisciplinary teams in settings like ICU, emergency room, outpatient clinics, or home & community care environments.
- Support infection control protocols and maintain sterile processing standards to ensure patient safety.
Experience
- Proven experience in diverse healthcare environments such as long term care and rehab.
- Skilled in managing complex cases including infectious disease care, pain management, behavioral health cases, and post-acute care scenarios.
- Prior work with special populations such as geriatrics (memory care)
- Proficiency in medical terminology and physiology knowledge essential for comprehensive patient assessment.
- Experience administering injections (including botulinum toxin treatments), performing venipuncture or catheterization procedures.

Pay: $30.00 - $35.00 per hour
Expected hours: 16.0 – 40.0 per week
