Job Summary We are seeking a dedicated Registered Nurse Case Manager to coordinate patient care within a dynamic healthcare environment. The ideal candidate will facilitate seamless discharge planning, manage utilization, and ensure optimal patient outcomes across various inpatient and outpatient settings. This role requires a strong understanding of hospital operations, medical terminology, and patient-centered care, particularly within trauma centers, ICU, and behavioral health units. The Hospital Case Manager will serve as a vital link between patients, families, healthcare providers, and community resources to promote effective recovery and post-acute care transitions.Duties<ul><li>Conduct comprehensive patient assessments to determine medical needs and discharge requirements.</li><li>Develop individualized care plans that include discharge planning, post-acute care arrangements, and resource coordination.</li><li>Collaborate with multidisciplinary teams across hospital departments such as ICU, PICU, trauma centers (Level I &amp; II), emergency medicine, and behavioral health units.</li><li>Manage utilization review processes to ensure appropriate level of care while complying with HIPAA regulations.</li><li>Coordinate with social workers, addiction counselors, and mental health professionals to support patients' psychosocial needs.</li><li>Facilitate communication between patients, families, insurance providers, and community agencies for smooth transitions of care.</li><li>Maintain accurate medical records and documentation related to patient care plans and case management activities.</li><li>Assist with intake procedures and diagnostic evaluations for inpatient and outpatient populations.</li><li>Support discharge planning efforts by arranging outpatient services, home health care, or other post-discharge resources.</li><li>Stay informed on hospital policies, managed care protocols, and relevant healthcare regulations to ensure compliance.</li></ul>Skills<ul><li>Graduate of a nursing program. BSN preferred.</li><li>Licensed Registered Nurse in the state of Pennsylvania.</li><li>Proven experience in hospital case management within acute care settings such as ICU, trauma centers (Level I &amp; II), or post-acute environments.</li><li>Strong knowledge of medical terminology, physiology, diagnostic evaluation, and patient assessment techniques.</li><li>Familiarity with HIPAA regulations and medical records management.</li><li>Experience in discharge planning, utilization management, and managed care processes.</li><li>Background in behavioral health or mental health counseling is advantageous.</li><li>Excellent communication skills for effective collaboration with multidisciplinary teams and patients&rsquo; families.</li><li>Ability to handle complex cases involving addiction counseling or social work interventions.</li><li>Proficiency in patient service delivery across inpatient and outpatient clinics; experience with pediatrics or post-acute care is a plus.</li><li>Strong organizational skills with attention to detail in documentation and case tracking.</li></ul>This position offers an opportunity to make a meaningful impact on patient recovery journeys by providing comprehensive case management within a highly specialized healthcare setting.Job Type: Full-timeBenefits:<ul><li>403(b)</li><li>Dental insurance</li><li>Health insurance</li><li>Paid time off</li><li>Vision insurance</li></ul>Work Location: In person
