Responsibilities
<ul><li>The Social Services Assistant works with the MSW Case Manager in identifying and providing for each resident&rsquo;s social, emotional, and psychological needs, and the continuing development of the resident&rsquo;s full potential during his/her stay to assist in the planning for his/her discharge.</li><li>
Develop a comprehensive social history assessment, a psycho-social assessment, discharge plan, and complete the MDS and Care Plan for new admits.</li><li>
Follow up and resolve any patient grievances.</li><li>
Facilitate the social and psychological well-being of residents and their families. Conduct a brief interview of Mental Health Status and Geriatric Depression Scale.</li><li>
Plan discharges/transfers to ensure appropriateness and continuity of care for transfers within and discharges from facility. Set up transportation as necessary, arrange Home Health services, and attend weekly Medicare meetings.</li><li>
Involve the entire facility in meeting residents&rsquo; psychosocial needs and make recommendations to staff where identified.</li><li>
Work cooperatively with supervisors and staff and maintains a good working relationship with all staff. Establishes good working relationships with all facility staff.</li><li>
Provide customer service to ensure internal and external customers of the facility have a positive experience.</li></ul>
Education/Experience: to perform this job successfully, an individual must have:
<ul><li>
Bachelor&rsquo;s Degree in Social Work or health related field; or two years of related experience in a health-care setting. Background and experience commensurate with long-term care services to the geriatric population, including ability to complete relevant sections of the Minimum Data Set (MDS).</li></ul> 


Certificates/Licenses: To perform this job successfully, an individual must:
