<p>Telos Health Systems is seeking an experienced <b>Physician Assistant (PA)</b> to serve as an in-house <b>SNF-ist</b>, providing general medicine care to geriatric residents in <b>Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs)</b>.</p>

</li></ul><p></p><h3 class="jobSectionHeader"><b>What You&rsquo;ll Do</b></h3>
<ul><li><p>Serve as the facility&rsquo;s primary internal medicine provider (SNF-ist).</p>
</li><li><p>Manage care for short-term rehab and long-term care residents.</p>
</li><li><p>Perform comprehensive health assessments, physical exams, and diagnoses.</p>
</li><li><p>Manage acute and chronic conditions following established protocols.</p>
</li><li><p>Prescribe medications and treatments as appropriate and within scope.</p>
</li><li><p>Conduct monthly stability visits for long-term residents.</p>
</li><li><p>Evaluate residents with changes in condition.</p>
</li><li><p>Participate in Advanced Care Planning.</p>
</li><li><p>Collaborate with interdisciplinary teams, medical directors, and attending physicians.</p>
</li><li><p>Complete accurate documentation and CPT coding.</p>
</li><li><p>Facilitate referrals to specialists and other healthcare providers.</p>

<p><b>Required:</b></p>
<ul><li><p>Active, unencumbered <b>Physician Assistant (PA)</b> license in the applicable state</p>
</li><li><p>Current DEA registration</p>
</li><li><p>Master&rsquo;s Degree from an accredited Physician Assistant program</p>
</li><li><p>Strong communication and clinical leadership skills</p>
</li><li><p>Ability to work effectively in a fast-paced environment</p>
</li></ul><p><b>Preferred:</b></p>
<ul><li><p>At least one year of experience in a Skilled Nursing Facility or long-term care setting</p>
</li><li><p>Experience caring for geriatric populations</p>
