Telos Health Systems is seeking an experienced Physician Assistant (PA) to serve as an in-house SNF-ist, providing general medicine care to geriatric residents in Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs) in the Glasgow, KY area.

</li></ul><h3 class="jobSectionHeader">What You&rsquo;ll Do</h3>
<ul><li>Serve as the facility&rsquo;s primary internal medicine provider (SNF-ist).
</li><li>Manage care for short-term rehab and long-term care residents.
</li><li>Perform comprehensive health assessments, physical exams, and diagnoses.
</li><li>Manage acute and chronic conditions following established protocols.
</li><li>Prescribe medications and treatments as appropriate and within scope.
</li><li>Conduct monthly stability visits for long-term residents.
</li><li>Evaluate residents with changes in condition.
</li><li>Participate in Advanced Care Planning.
</li><li>Collaborate with interdisciplinary teams, medical directors, and attending physicians.
</li><li>Complete accurate documentation and CPT coding.
</li><li>Facilitate referrals to specialists and other healthcare providers.
</li></ul><h3 class="jobSectionHeader">What You&rsquo;ll Need</h3>
Required:
<ul><li>Active, unencumbered Physician Assistant (PA) license in the applicable state
</li><li>Current DEA registration
</li><li>Master&rsquo;s Degree from an accredited Physician Assistant program
</li><li>Strong communication and clinical leadership skills
</li><li>Ability to work effectively in a fast-paced environment
</li></ul>Preferred:
<ul><li>At least one year of experience in a Skilled Nursing Facility or long-term care setting
</li><li>Experience caring for geriatric populations
