Position Overview

We are seeking a compassionate and motivated Post-Acute Care Physician (MD/DO) to provide on-site primary care services within skilled nursing facilities throughout the Tampa area.

This opportunity is ideal for physicians seeking a flexible schedule, strong clinical support, and the ability to make a meaningful impact on patients transitioning through post-acute rehabilitation and long-term care. Clinical Responsibilities

  • Provide comprehensive primary care services to residents in skilled nursing facilities
  • Evaluate, diagnose, and manage acute and chronic medical conditions
  • Conduct admissions, follow-up visits, and medically necessary evaluations
  • Assess and manage residents experiencing changes in condition
  • Collaborate with nursing staff, therapists, specialists, and facility leadership
  • Supervise and support NP/PA clinicians as appropriate
  • Participate in quality and patient care initiatives

After-Hours Coverage

  • Participate in a structured and manageable call rotation
  • Address urgent clinical concerns through telehealth services
  • Provider-delivered telehealth encounters are billable
  • Supported processes designed to maintain reasonable call volume

Compensation

Pay-Per-Encounter Model

  • Competitive compensation for each patient encounter
  • Earnings supported by stable facility census and visit volume

Qualifications

  • MD or DO degree required
  • Board Certified or Board Eligible in:
    • Internal Medicine
    • Family Medicine
    • Geriatrics
    • Emergency Medicine
  • Active Florida medical license (or ability to obtain)
  • DEA registration (or ability to obtain)
  • Interest in post-acute care, skilled nursing medicine, and collaborative patient care
