Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Primary Care Physician for Element Care PACE in Lynn, Massachusetts.
POSITION OVERVIEW
-
Provide comprehensive primary care for medically complex older adults enrolled in the Program of All-Inclusive Care for the Elderly (PACE).

Manage acute illnesses, chronic diseases, preventive care, medication management, and annual wellness evaluations.

Conduct visits in the PACE center, participant homes, skilled nursing facilities, and through telehealth when appropriate.

Serve as the physician leader within an interdisciplinary care team alongside Nurse Practitioners, nurses, therapists, social workers, pharmacists, and care coordinators.

Complete hospital follow-up visits and coordinate care transitions to reduce avoidable admissions.

Support participant-centered goals that allow seniors to remain safely living within the community.

Competitive salary from $200,000-$294,000 based on experience.

Requirements:
MINIMUM REQUIREMENTS
-
Board Certified or Board Eligible in Internal Medicine or Family Medicine.

Eligible for unrestricted Massachusetts medical license.

Active DEA registration or ability to obtain.

Experience or strong interest in geriatrics, primary care, or complex chronic disease management.

Excellent communication and interdisciplinary collaboration skills.

Commitment to patient-centered, community-based senior care.
