Job Summary We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our healthcare team. The LVN will provide high-quality patient care across various settings, including acute, long-term, and community-based care environments. This role requires a strong foundation in nursing practices, excellent assessment skills, and the ability to work effectively with diverse patient populations, including seniors, individuals with developmental disabilities, and those requiring specialized care such as Alzheimer's or memory care. The ideal candidate will have experience with EMR/EHR systems, medication administration, and vital signs monitoring, ensuring compliance with healthcare standards and patient safety protocols.Duties<ul><li>Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using EMR/EHR systems such as Epic and Cerner.</li><li>Administer medications and injections accurately following physician orders, including IV infusions and catheterizations.</li><li>Monitor patients&rsquo; conditions closely in various settings such as ICU, emergency room, medical-surgical units, hospice care, or outpatient clinics.</li><li>Assist with complex procedures like airway management, ventilator care, spinal taps, and cardiac catheterization.</li><li>Provide wound care, sterile processing, suture removal, and aseptic techniques to prevent infection.</li><li>Conduct diagnostic evaluations such as laboratory specimen collection/processing and medical imaging procedures.</li><li>Educate patients and families on health coaching topics including chronic disease management (e.g., diabetes, hypertension), nutrition, discharge planning, and post-acute care.</li><li>Collaborate with multidisciplinary teams in case management and discharge planning to ensure seamless patient transitions.</li><li>Maintain accurate medical documentation adhering to HIPAA regulations and ICD coding standards (ICD-9/ICD-10).</li><li>Participate in infection control practices and adhere to NCQA standards for quality patient care.</li></ul>Requirements<ul><li>Valid LVN license with current state licensure.</li><li>Proven experience in diverse healthcare environments such as ICU (Level I &amp; II Trauma Centers), emergency medicine (urgent care/emergency room), geriatrics (long-term care/dementia/memory care), or home &amp; community care.</li><li>Proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth, or eClinicalWorks.</li><li>Strong knowledge of physiology, anatomy, pharmacology (including medication administration), and medical terminology.</li><li>Experience with specialized procedures like IV insertion/infusion, phlebotomy/venipuncture, catheter care/tube feeding.</li><li>Ability to work effectively with patients of all ages&mdash;including pediatrics (neonatal ventilator/infant care) and adults&mdash;especially those with disabilities or complex health needs such as Alzheimer's or infectious disease care.</li><li>Critical thinking skills for triage assessments and acute pain management.</li><li>Excellent communication skills for patient education and interdisciplinary collaboration.</li><li>Ability to work independently while following protocols for infection control and aseptic technique.
