This role is ideal for a professional with strong knowledge of community resources and social service systems who is passionate about helping individuals and families access essential support services and maintain stability.Key Responsibilities:The Licensed Clinical Social Worker (LCSW) will assist clients in accessing and maintaining benefits and services through agencies such as HPD and HRA. Responsibilities include conducting client assessments, developing individualized service plans, and providing ongoing case management support. The LCSW will help clients navigate housing programs, public assistance, and other social service resources while ensuring all documentation is accurate and up to date. This role also includes coordinating with city agencies, advocating on behalf of clients, monitoring case progress, and ensuring compliance with all program and regulatory requirements.
