The Primary Care NP provides comprehensive medical management to residents within Assisted Living Program (ALP) facilities. Unlike a traditional clinic, this role brings the &quot;Doctor&rsquo;s Office&quot; to the resident. You will manage a dedicated panel of patients across multiple sites in the Utica/Rome area, focusing on &quot;treating in place&quot; to reduce unnecessary ER transfers and improve the quality of life for the geriatric population.<h3 class="jobSectionHeader">
Key Responsibilities</h3><ul><li>
Clinical Management: Perform comprehensive physical exams, diagnostic screenings, and acute &quot;sick visits&quot; at the patient&rsquo;s bedside within the ALP.</li><li>
Chronic Disease Oversight: Manage complex geriatric conditions including COPD, CHF, Diabetes, and Cognitive Impairment (Dementia/Alzheimer's).</li><li>
Travel &amp; Coverage: Travel daily between assigned facilities within the Utica cluster (typically 2&ndash;4 sites depending on census).</li><li>
Medication Management: Conduct regular medication reviews to reduce polypharmacy and ensure compliance with NYS Department of Health (DOH) regulations for ALPs.</li><li>
Care Coordination: Act as the primary liaison between facility staff (Directors of Nursing), families, and outside specialists or hospice providers.</li><li>
Documentation: Maintain timely and accurate EMR records (Electronic Medical Records) to meet NYS Medicaid and Medicare billing requirements.</li></ul><h3 class="jobSectionHeader">
Required Qualifications</h3><ul><li>
Licensure: Active New York State Nurse Practitioner license (FNP, ANP, or AGNP) in good standing.</li><li>
Certifications: Board Certification (AANP/ANCC) and active DEA registration.</li><li>
Experience: Minimum of 1&ndash;2 years in Primary Care, Geriatrics, or Long-Term Care is preferred.</li><li>
Mobility: Valid NYS Driver&rsquo;s License and a reliable vehicle for travel within the Central NY region.</li><li>
Compliance: Active NYS Medicaid Provider ID (or eligibility to obtain one).</li></ul><h3 class="jobSectionHeader">
Preferred Skills</h3><ul><li>
Strong interpersonal skills for navigating the social dynamics of assisted living environments.</li><li>
Ability to work autonomously with remote physician collaboration.</li><li>

Regional Geography: The Utica Cluster</h3>
The &quot;Utica Cluster&quot; generally includes facilities in the following areas:<ul><li>
Oneida County: Utica, Rome, New Hartford, Whitesboro, and Clinton.</li><li>
Herkimer County: Herkimer, Little Falls, and Ilion.</li></ul>
