Primary Care Nurse Practitioner (Mobile)

Role Overview

The Primary Care NP provides comprehensive medical management to residents within Assisted Living Program (ALP) facilities. Unlike a traditional clinic, this role brings the "Doctor’s Office" to the resident. You will manage a dedicated panel of patients across multiple sites in the Utica/Rome area, focusing on "treating in place" to reduce unnecessary ER transfers and improve the quality of life for the geriatric population.

Key Responsibilities

  • Clinical Management: Perform comprehensive physical exams, diagnostic screenings, and acute "sick visits" at the patient’s bedside within the ALP.

  • Chronic Disease Oversight: Manage complex geriatric conditions including COPD, CHF, Diabetes, and Cognitive Impairment (Dementia/Alzheimer's).

  • Travel & Coverage: Travel daily between assigned facilities within the Utica cluster (typically 2–4 sites depending on census).

  • Medication Management: Conduct regular medication reviews to reduce polypharmacy and ensure compliance with NYS Department of Health (DOH) regulations for ALPs.

  • Care Coordination: Act as the primary liaison between facility staff (Directors of Nursing), families, and outside specialists or hospice providers.

  • Documentation: Maintain timely and accurate EMR records (Electronic Medical Records) to meet NYS Medicaid and Medicare billing requirements.

Required Qualifications

  • Licensure: Active New York State Nurse Practitioner license (FNP, ANP, or AGNP) in good standing.

  • Certifications: Board Certification (AANP/ANCC) and active DEA registration.

  • Experience: Minimum of 1–2 years in Primary Care, Geriatrics, or Long-Term Care is preferred.

  • Mobility: Valid NYS Driver’s License and a reliable vehicle for travel within the Central NY region.

  • Compliance: Active NYS Medicaid Provider ID (or eligibility to obtain one).

Preferred Skills

  • Strong interpersonal skills for navigating the social dynamics of assisted living environments.

  • Ability to work autonomously with remote physician collaboration.

  • Proficiency in wound care, minor procedures, and palliative care discussions.

Regional Geography: The Utica Cluster

The "Utica Cluster" generally includes facilities in the following areas:

  • Oneida County: Utica, Rome, New Hartford, Whitesboro, and Clinton.

  • Herkimer County: Herkimer, Little Falls, and Ilion.

Your role is crucial in helping residents maintain their independence. You are often the highest-level medical decision-maker on-site.
