About the Role
We are a small, independent home-based primary care practice serving older adults in the Bangor area.
We’re looking for a Nurse Practitioner who values time with patients, clinical autonomy, and relationship-based care—and who is interested in helping shape a growing practice in a meaningful way.
This is not a high-volume clinic role. Care is delivered where patients live, often with families and caregivers involved, focusing on function, comfort, stability, and continuity.
What You’ll Do
- Provide primary care in patients’ homes and assisted living settings
- Manage complex chronic conditions in older adults
- Support transitions of care between hospital, rehab, and home
- Work closely with families, caregivers, and community supports
- Participate in care planning and ongoing care coordination
- Help refine workflows that support safe, thoughtful home-based care
- Optional: develop areas of interest such as dementia care, palliative care, or chronic care management

Compensation
A transparent, production-based model:
- Compensation tied to patient visits and collected revenue
- Shared practice infrastructure (EHR, billing, admin support) ~9% of collections
- Shared medical assistant support
- Income increases with panel growth and participation in care programs (CCM, AWV, TCM)

- Nurse Practitioner with interest in geriatrics or adult primary care
- Comfortable working independently in home and community settings
- Strong clinical judgment and communication skills
- Values thoughtful, unhurried care
- Enjoys working closely with older adults and families
- Interested in helping build and improve a small practice over time
