This position offers an exciting opportunity to work in a fast-paced environment where your skills in acute and chronic care, medical documentation, and patient education will make a meaningful difference.Responsibilities<ul><li>Conduct thorough physical examinations and health assessments.</li><li>Develop and implement individualized treatment plans, including medication administration, injections.</li><li>Manage acute and chronic conditions.</li><li>Collaborate with multidisciplinary teams to coordinate discharge planning, case management, utilization review, and telehealth services ensuring comprehensive patient care.</li><li>Maintain accurate medical records using EMR/EHR eClinicalWorks while adhering to HIPAA regulations.</li><li>Provide patient education on health coaching topics such as nutrition, medication management, disease prevention (including infectious disease care), and lifestyle modifications.</li></ul>Skills<ul><li>Extensive experience with EMR/EHR systems eClinicalWorks.</li><li>Strong knowledge of physiology, anatomy and medical terminology relevant to primary care.</li><li>Familiarity with CPT coding (Current Procedural Terminology), ICD-10 coding for accurate documentation and billing processes.</li></ul>Pay: $50.00 - $60.00 per hourBenefits:<ul><li>Flexible schedule</li></ul>Work Location: In person
