Nurse Practitioner

Job Overview

Responsibilities

• Conduct thorough physical examinations and health assessments.
• Develop and implement individualized treatment plans, including medication administration, injections.
• Manage acute and chronic conditions.
• Collaborate with multidisciplinary teams to coordinate discharge planning, case management, utilization review, and telehealth services ensuring comprehensive patient care.
• Maintain accurate medical records using EMR/EHR eClinicalWorks while adhering to HIPAA regulations.
• Provide patient education on health coaching topics such as nutrition, medication management, disease prevention (including infectious disease care), and lifestyle modifications.

Skills

• Extensive experience with EMR/EHR systems eClinicalWorks.
• Strong knowledge of physiology, anatomy and medical terminology relevant to primary care.
• Familiarity with CPT coding (Current Procedural Terminology), ICD-10 coding for accurate documentation and billing processes.

Pay: $50.00 - $60.00 per hour
