Summary

Northeast Michigan Community Mental Health Authority (NeMCMHA) is seeking skilled Case Managers to support individuals across our four‑county region. This role plays a key part in helping people with serious mental illness, intellectual and developmental disabilities, autism spectrum disorder (ASD), emotional disturbance, and substance use disorders access the services they need to live as independently as possible.

Key Responsibilities

  • Conduct assessments and develop person‑centered service plans
  • Coordinate care and link individuals to appropriate supports
  • Monitor progress and adjust services as needed
  • Provide crisis intervention and participate in an on‑call rotation
  • Collaborate with families, schools, jails, and community partners
  • Support individuals in the least restrictive, most integrated settings

Qualifications

  • Bachelor’s degree in Social Work, Education, or Nursing
  • State registration as an RN or LBSW (or eligibility to obtain)
  • Minimum of one year of experience in mental health or nursing
  • Lived experience related to mental illness, developmental disabilities, or substance use valued
  • Strong communication skills and functional computer literacy (within 6 months of hire)
  • Valid driver’s license with acceptable driving record
