In this role, you’ll support individuals navigating mental health and substance use challenges by meeting them where they are—literally and figuratively. Through personalized, evidence‑based support, you’ll empower clients to develop skills, access resources, and achieve stability and long‑term recovery.

What You’ll Do

  • Provide community‑based mental health support, including assessment, treatment planning, and interventions

  • Partner with clients and families to create individualized, recovery‑focused plans

  • Use a strengths‑based, trauma‑informed approach incorporating Motivational Interviewing, CBT, DBT skills, and positive reinforcement techniques

  • Teach and model coping skills, psychiatric self‑management, and life skills

  • Coordinate care with healthcare providers, schools, and community organizations

  • Facilitate PSR groups aligned with client treatment goals

  • Maintain accurate and timely client documentation

  • Collaborate closely with a multidisciplinary care team to ensure comprehensive support

  • Participate in training, supervision, and quality improvement initiatives

  • For ACT/ACT TAY/IMPART roles: Apply knowledge of the ACT model and core service delivery processes

  • Perform other related duties as assigned

What We’re Looking For

You qualify if you have one of the following:

  • Bachelor’s degree in Psychology, Social Work, or a related human services field, or

  • Bachelor’s degree in an unrelated field plus two (2) years of relevant experience, or

  • A combination of four (4) years of education and experience, or

  • Associate of Applied Science in Behavioral Health Support, or

  • Four (4) years of qualifying experience*

  • Qualifying experience includes direct service with individuals experiencing mental health disorders, substance use disorders, or developmental disabilities.

Relevant experience includes:

  • Providing individual or group rehabilitation/habilitation services

  • Supporting recovery, resiliency, and skill development

  • Assisting individuals with employment, education, family, and community engagement

  • Helping clients achieve goals outlined in individualized treatment plans

Skills & Qualities for Success

  • Knowledge of case management and behavioral health treatment principles

  • Familiarity with community resources and mental health systems

  • Strong written and verbal communication skills

  • Ability to build trusting, meaningful relationships

  • Self‑directed, organized, and adaptable

  • Comfortable working independently while staying connected to a team

  • Open to supervision, feedback, and ongoing learning
