QUALIFICATIONS
Preferred experience of 1 year or more with reception or clerical and medical records responsibilities; The position requires an emphasis on customer service, phone skills, and organizational and time management.
RESPONSIBILITIES:
The Administrative Assistant will be responsible for office organization and ensuring medical documentation is completed within the patients clinical record in a timely manner.
The Administrative Assistant works with the Executive Director and in conjunction with the Interdisciplinary Team to: Answers phone promptly and courteously and directs messages as indicated. Provides phone coverage for all locations, maintains contact lists, updates local and home office on staff changes.
- Complete and report daily census utilizing provided template.
- Analyzes and reviews medical records for completeness and accuracy of documentation according to regulatory standards and company policies.
- Confers with doctors, nurses, and other team members to assure complete, current, and accurate medical records.
- Compiles, maintains, and reviews logs, reports, and statistical records, and researches records to locate health data as requested.
- Review of the Patient Medical Record (ongoing) in accordance with audit sheets to ensure chart completeness.
- Completes and coordinates monthly billing audit tools.
- Creates, coordinates, and maintains recert/F2F documents in the electronic medical record.
- Schedules routine visits for all disciplines, including required tasks for admissions, referrals and re-certifications in the electronic medical record.
- Review and process physician orders to ensure signatures are obtained and completion of the medical record. Maintains patient forms/records including, but not limited to, legals, consent, H&P, Advance Directives, Death Certificates.
- Assists ED/DCS in facilitating referrals and admission process, completing the referral intake sheet, verifying insurance, and obtaining appropriate documentation as needed.
- Maintains the medical record to ensure adequate documentation is completed on all patients.
- Reviews and maintains reports including, but not limited to, past due visits report, schedule report and schedule deviation report.
- Ensures the pharmacy reports are uploaded into the medical record.
- Creates agenda for IDT and ensures that scheduling requirements are met in the EMR
- Creates the On Call schedule and ensures after-hour calls are covered.
- Maintains the Education spreadsheet and tracks all education for each staff member.
- Maintains and ensures personnel files are complete with current documentation at all times.
- Assists with employee orientation and onboarding.
- Sets up new employee equipment: and IT ticketing as needed tracking assigned equipment on spreadsheet per protocol.
- Maintains Community Outreach Spreadsheet as indicated.
- Updates and maintains the Emergency/Disaster Binder
- Clerical Functions: Including, but not limited to, faxing, filing, sorting mail, scanning invoices as necessary.
- Orders office supplies, branch level marketing materials, and name badges.
- Maintains MSDS Binder
- Implements practices that ensures Family Satisfaction measures exceed national benchmarks
- Processes donations as necessary
- Other responsibilities as assigned

- Adheres to all HIPPA, HITECH, EPHI and company policies and procedures regarding patient protected information.
ORIENTATION AND TRAINING
Hospice will provide orientation and training, as necessary, to assist Administrative Assistant in performing the responsibilities listed above. Training may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file.
JOB REQUIREMENTS:
Job requires the ability to:
- Communicate effectively with patient/families both verbally and written
- Communicate the mission and vision of the hospice
WORK CONDITIONS;
May require sitting or standing for prolonged periods of time. Driving/travel required. May require occasional lifting, pushing or pulling up to 50 lbs.
ACCOUNTABILITY
Reports directly to the Executive Director.
