Job Summary:
The Administrative Assistant/Targeted Case Manager provides comprehensive administrative support while coordinating case management services for individuals with developmental disabilities and families. This role is responsible for maintaining accurate records, managing correspondence, and ensuring efficient program operations. In addition, the position assesses individual needs, develops individualized service plans, coordinates referrals to community resources, monitors progress, and advocates for access to appropriate services. The Administrative Assistant/Targeted Case Manager works collaboratively with persons supported, healthcare providers, community support providers, and direct support professionals to promote positive outcomes while maintaining compliance with organizational policies, confidentiality standards, and applicable state and federal regulations. Strong organizational, communication, and problem-solving skills are essential to effectively balance administrative responsibilities with person-centered case management services.
