Assistant, Case Management

The Legacy Clinic is a new and unique functional medicine practice in Cody Wyoming, dedicated to improving the health and longevity of our patients. We are seeking a dedicated and results-oriented individual to join our team as a Case Manager.

Job Summary: The Case Manager is instrumental in building and maintaining strong patient relationships while driving clinic growth. This role involves conducting initial consultations, gathering patient information, and guiding patients through their healthcare journey. Your primary focus will be converting leads into long-term patients by providing outstanding customer service and ensuring a seamless patient experience.

Responsibilities:

Patient Intake and Assessment:

• Conduct comprehensive initial consultations with new leads to identify their healthcare needs.
• Gather detailed patient information through health questionnaires and other assessments.
• Prepare and review patient paperwork to ensure accuracy and completeness.
• Coordinate patient hand-off to health providers for examinations.

Patient Care Coordination:

• Develop and manage patient treatment plans in collaboration with health providers.
• Handle patient finances, scheduling, and agreement processes efficiently.
• Provide exceptional customer service by addressing patient inquiries and concerns promptly.
• Maintain clear and detailed patient records.

Sales and Conversion:

• Effectively communicate the benefits of our services to potential patients.
• Build rapport and trust with leads to foster long-term relationships.
• Utilize sales techniques to convert leads into patients.
• Adhere to company sales guidelines and standards.

Qualifications:

• CNA, LPN, or similar required
• Strong interpersonal and communication skills
• Excellent organizational and time management abilities
• Ability to build rapport and trust with patients
• Basic understanding of healthcare and sales principles
• A passion for helping others
• Phlebotomy experience preferred
