RESPONSIBILITIES:
  • Develops and maintains professional working relationships with referring facilities, agencies, physicians and the community.
  • Conducts visits to local referring agencies marketing Northeast Center Services and Programs. Promotes awareness about service (type, scope and quality) that is offered.
  • Attends Department Director Meetings as required. Coordinates relevant meetings with Department Directors and Consultants.
  • Participation in Facility committees as assigned.
  • Participates in the Facility Quality Improvement Program.
  • Ensures accurate data collection and analysis.
  • Direct, monitors and evaluates the performance of the Assessment/Screeners.
  • Receives incoming calls from Screeners, Social Workers, Discharge Planners and Family members regarding all potential admissions. Maintains accurate records of referrals according to
  • Facility policy.
  • Coordinates the completion of all pre-admission evaluations on all potential residents. Assesses medical and financial eligibility for admission. Make professional decisions regarding clinical assessment, data analysis, problem solving and projection of resident outcomes when applicable. Collaborates with the Assessment/Screening Nurse on findings and decisions. Based upon these decisions, making decisions about admission involves appropriate administrative personnel when needed.
  • Communicates to the referring agency all decisions regarding admissions within twenty-four hours. (NBI admissions when documentation is complete and reviewed by Director of Nursing)
  • Arranges for resident/family interviews and tours and provides information as needed to the applicant or family members.
  • As necessary, assists the Assessment/Screening Nurse in facilitating resident admission from the referral source and distribute information to key areas within the Facility prior to admission.
  • Assesses medical coverage and ensures that all necessary special equipment is available at the time of admission.
  • Ensures that all information needed is received from families and/or responsible parties prior to or on the day of admission.
  • Submits all information to the Medical Records Department upon admission.
  • Develops a working network of peers by attending meetings and seminars appropriate to job function.
  • Collaborates with Central Marketing personnel regarding census, referrals, and Marketing Programs.
  • Collaborate with Community Re-entry Director regarding documentation and needs of TBI waiver admissions.
  • Assesses referrals that meet services provided by USG facilities and with permission initiates the process of forwarding said referral.
  • Assesses bed availability and projects changes to meet the present market, initiates the necessary changes.
  • Performs other duties as assigned.

EDUCATION/EXPECTATIONS

  • Graduate of an accredited School of Nursing with a current New York license as a Registered Nurse
  • Minimum two (2) years Clinical Nursing experience.
  • Independently motivated, capable of working well with minimum supervision.
  • Excellent verbal and written assessment and communications skills.
  • Treats residents/patients with respect, dignity, and compassion.
