Job Overview:
The Nurse Practitioner for the Pulmonary Service works collaboratively within a multidisciplinary health team and is responsible for providing comprehensive and age appropriate direct care and follow up to a population of patients. The components of the role include clinical practice, consultation, professional development, education, and research. The Pulmonary Nurse Practitioner works in conjunction with the Pulmonary Attendings performing patient consults and follow up care. Clinical core privileges will include the management, treatment, and ordering, performing, and interpreting labs, radiography, and other diagnostics test for acute and chronic pulmonary and sleep disorders of the adult population.

Essential Functions:

Utilizes advanced assessment skills to manage a cohort of patients

Performs and documents assessments
-
Collects and analyzes data and formulates a patient-centered plan of care

Evaluates and monitors the effectiveness of the prescribed treatment plan
-
Using the most current evidence modifies the plan of care based on the effectiveness of the treatment and the patient/family response to the plan
-
Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities

Serves as an educational resource to the patient/family, nursing staff, and provider team
-
Acts as a consultant to multidisciplinary health care team members

Educates patients/families about self-management of acute or chronic illnesses, tailoring instructions to patients individual circumstances

Facilitates the patients transition within and between health care settings admitting, transferring, and discharging patients when appropriate

Demonstrates a commitment to patient safety by speaking up about safety issues and changing practice to enhance safety,

Shares errors or potential errors through system reporting mechanisms

Demonstrates shared accountability for safe practice

Establishes and maintains communication with other health care providers and departments to ensure operational efficiency and effectiveness and to promote consistency and continuity of care
